What the inspector wrote

Licensing Program Analyst (LPA), Brittany Lovest and Keyona Scott, conducted an unannounced Case Management - Incident inspection at the Child Care Center on 04/1/2025, for the purpose of investigating an incident that occurred on 03/11/2025 between staff and a child. LPAs met with Principal Desiree Hall at 10:42 AM. LPAs were guided on a tour inside and outside of the facility. LPAs observed the following appropriate teacher to child ratios at 10:52 AM: sixteen (16) children to one (1) teacher and two (3) Aides in Room 1; fourteen(14) children to one (1) teacher and three (3) Aides in Room 2; twenty-one (21) children to one (1) teacher and three (3) Aides in Room 3/PCC; eight(8) children to one (1) teacher and three (3) Aides for Room 4, seventeen (17) children to one (1) teacher and three (3) Aides for Room 5. During census LPAs observed Room 3 on the outdoor north play yard and Room 2 on the outdoor east play yard. As a condition of employment, all individuals working or present at the facility that is operated by a school district shall obtain a criminal record clearance or criminal record exemption. The school district shall notify the Regional Office, if there is a change of status of an individual\342\200\231s criminal record clearance. During today's inspection, LPAs obtained the following documents: Written statements provided by principal, Child Care Facility Roster (LIC 9040), copy of Classroom Sign-in sheet for 3/10/2025- 3/14/2025 for Room 1, copy of LAUSD Discipline Foundation Policy. During today's inspection, LPAs conducted interviews and facility observation. Based on information obtained, incident requires further investigation. A notice of site visit was given and must remain posted for 30 days. Exit interview conducted and report was reviewed with Principal, Desiree Hall.
