What the inspector wrote

On 11/13/24 at 9:30 am, LPA Licensing Program Analysts (LPA) Claudia Kam conducted a case management visit at the above facility for the purpose of case management to follow up on unusual incident reports reported to the department. Upon arrival, LPA met with Rosa Ramirez, principle and provided LPA a tour of the facility. LPA observed proper care and supervision. During this inspection LPA interviewed Facility Representative, who reported the incident to Licensing, and she stated that she followed LAUSD protocols in calling Law Enforcement Agency (HPPD), submitted SCAR report, and informed LAUSD Operations. HPPD notified Facility Representative to handle Administratively, and no investigation was conducted by them. Facility Representative states she has conducted investigation which involves interviewing staff and children. Per Facility Representative, interviews did not provide corroborating information. Facility Representative states the investigation is complete and inconclusive. Representative states due to this incident, she has attempted to conduct a behavior assessment with no results and will do an internal assessment for child. Regarding staff a discussion has been had, regarding Personal Rights for children and Code of Ethics. LPA conducted interviews with staff present and involved in the incidents. Based on the information obtained, there was no violation of personal rights at the facility and not current suggestions for correction needed. The facility was found in compliance per Title 22 regulations, there will be no deficiencies cited today. A notice of site visit was given and must remain posted for 30 days. Exit interview was conducted and report was reviewed with the Facility Representative, Rosa Ramirez.
