What the inspector wrote

On 10/21/25 at 12:25 PM Licensing Program Analyst (LPA) Claudia Kam conducted an Unannounced Case management Inspection to follow up on an incident that was reported to the Department. Upon arrival, LPA announced purpose of inspection and met with Facility Representative, Principle Rosa Ramirez who granted entry to facility. Census was taken. On 10/9/2025 and 10/15/2025, an incident was reported to The Department where a child was left without supervision. The facility reported this incident to the Department within the required 24 hours and submitted written report within seven days. During this inspection LPA interviewed staff, observed classrooms and children named in reports. S1,S2,S3, S4, S5 confirmed that on both occasions a child left the classroom without staff supervising and without the staff knowing that they had left the classrooms. Children were not alone in either incident with staff nearby to observe the children. C1 left the room to play outside with the classrooms participating in outdoor play. C2 left to use the restroom and an aide was present to observe her and return her to the classroom. Children were observed by S1- S5 to be unfazed by the incidents and were returned immediately after departure from the classrooms. FR has made changes to supervision, ensuring that the doors are locked so that children who have a tendency to leave a classroom cannot and a strategy has been put in place to ensure that there is supervision during educational time and nap time. FR and staff are communicating with the parent of C1 to create a plan as part of the correction. Due to children starting school at the facility within the last 30 days staff are learning the behaviors and needs of the children in the classroom. Page 1 of 2 Based on the information obtained and LPA observation LPA has determined that although children were not with their assigned staff there was supervision and Facility Representative has made necessary adjustments after the incidents. The following deficiencies on the LIC 809 D are being cited today 10/21/25. A notice of site visit was given and must remain posted for 30 days.

Interviews confirmed punishment of child. This poses an possible health, safety, and personal rights risk to children in care.
