What the inspector wrote

On 10/21/2025 at 9:45 AM Licensing Program Analyst (LPA) Claudia Kam conducted an Unannounced Case Management inspection to follow up on an incident that was reported to The Department on 8/15/2025. Upon arrival, LPA met with Facility Representative (FR) Principal Rosa Ramirez. Census was taken. An incident was reported to The Department in regards to personal rights. The facility reported this incident to the Department within the required 24 hours and submitted a written report via email within seven days, complying with Title 22 Regulation requirements. During this inspection LPA interviewed FR, staff involved, and child. FR followed LAUSD protocols regarding these types of incidents, reporting to HPPD Child Abuse Unit, reporting Operations Coordinator and writing an ISTAR and SCAR report. Facility Representative was instructed by HPPD to handle administratively. Facility Representative states that internal investigations are complete which included interview of children and obtaining written statements from Staff. FR states that no medical care was required for incident and children's Authorized Representatives were notified. LPA interviewed staff involved in the incidents and interviews confirm that a child was hit by staff on the forehead. C1 Interview did not confirm allegation as he is not fully verbal. S1 stated that S3 vocalized that she used a child's hand to hit child C1 while S2 stated that she witnessed S3 hit the child on the forehead and was observed checking the child's forehead throughout the day. While the statements are varied it was consistent that the child was hit. Page 1 of 2 Based on the information the following deficiencies on the LIC 809 D are being cited today 10/21/25. A notice of site visit was given and must remain posted for 30 days. Exit interview conducted and report was reviewed with the principle Rosa Ramirez.

Interviews confirmed punishment of child. This poses an possible health, safety, and personal rights risk to children in care.
