What the inspector wrote

On 2/19/2026 at 9:00 AM Licensing Program Analysts (LPAs) Claudia Kam and Erica Lujan conducted an Unannounced Case Management inspection to follow up on an incident that was reported to The Department on 1/30/2026 and 10/22/25. Upon arrival, LPA met with Facility Representative (FR) Principal Rosa Ramirez. Census was taken. An incident was reported to The Department in regards to personal rights. The facility reported this incident to the Department within the required 24 hours and submitted a written report via email within seven days, complying with Title 22 Regulation requirements. During this inspection LPAs interviewed FR and staff involved, children were not present. FR followed LAUSD protocols regarding these types of incidents, reporting to HPPD Child Abuse Unit, reporting Operations Coordinator and writing an ISTAR and SCAR report. Facility Representative was instructed by HPPD to handle administratively. Facility Representative states that internal investigations are complete regarding the reported incidents which includes interviews of children and obtaining written statements from Staff. FR states that no medical care was required for incidents and children's Authorized Representatives were notified. LPAs interviewed staff involved in the incidents and interviews confirm the allegations that a child's personal rights were violated for incident on 1/30/26 and 10/22/25. Staff 1, Staff 2, Staff 4, and Staff 5 confirmed that a child on 1/30/26 asked to go to the bathroom during rug time and was told no that the child needed to wait by Staff 3. Staff 1 confirmed that on 10/24/25 Staff 3 during rug time took toys away from children and threw them across the room hitting a staff during one instance and one hit the ceiling on the second instance. Internal investigation by the principle has substantiated both instances. Page 1 of 2 Based on this information the following deficiencies on the LIC 809 D are being cited today 2/19/26. A notice of site visit was given and must remain posted for 30 days. Exit interview conducted and report was reviewed with the principle/facility representative Rosa Ramirez. Page 2 of 2

Interviews with S1, S2, S4, S5 confirmed that S3 told the child that they cannot use the bathroom. Staff took toys from a child and threw them hitting staff and ceiling. This poses an possible health, safety, and personal rights risk to children in care.
