Case management incident follow-up for personal rights violation from 3/27/25. Investigation found that staff member threatened to throw away child's shoes and threatened to call parent. Facility reported incident timely and followed LAUSD protocols. Deficiency citations issued.

Case management follow-up on two reported incidents (02/12 and 02/27/2025) involving supervision gaps and child injury. Facility submitted timely incident reports and developed adequate action plans for improved supervision both indoors and outdoors.

The school-age center was operating within capacity and had clean, organized classrooms with water readily available and active COVID-19 procedures in place. The analyst confirmed the carbon monoxide detector was operable, but found that all 14 staff files were missing immunization records.
