What the inspector wrote

On 10/01/25 Licensing Program Analyst (LPA) Ranita Richmond conducted an unannounced case management incident inspection at Westminster Ave EEC, located in Venice for the purpose of following up on the unusual incident that was self-reported by the facility. The El Segundo Child Care Regional Office received the report on 9/16/2025. Upon arrival, LPA met with Principal Felicia Torrence and discussed the purpose of the visit. LPA toured the facility inside the classrooms and outside on the playgrounds. LPA observed 40 children in care with 13 staff providing care and supervision. According to the incident report, on 9/16/2025, on 9/16/2025 during outside play while on the play yard playing soccer with children the staff tripped over her feet and fell to the ground hurting her right ankle and left wrist. During today’s inspection, LPA called teacher, no answer, left voice message to call LPA. LPA interviewed Principal. Per Principal, the teacher is out on workman’s compensation. There is no current date of return. Per the report, no children were near the teacher or hurt at the time of the incident. At this time, no further investigation is necessary. Per Title 22, Division 12, Chapter 3, of the California Code of Regulations, no deficiencies cited today. An exit interview was conducted and a copy of this report along with the Notice of Site Visit were provided to Felicia Torrence, Principal.
