On 6/26/2025 at 11:05 AM Licensing Program Analyst (LPA) Claudia Kam conducted an Unannounced Case Management inspection to follow up on an incident that was reported to The Department on 4/23/25. Upon arrival, LPA met with Facility Representative (FR) Principal Kandis Hamblet. Census was taken. On 4/23/25, an incident was reported to The Department in regards to personal rights. The facility reported this incident to the Department within the required 24 hours and submitted a written report via email within seven days, complying with Title 22 Regulation requirements. During this inspection LPA interviewed FR, and staff, child involved was not present. FR followed HPPD protocols regarding these types of incidents, reporting to LAPD Child Abuse Unit, reporting to Operations Coordinator and writing an ISTAR and SCAR report. Facility Representative was instructed by HPPD to handle administratively. Facility Representative states that internal investigations was completedregarding the reported incident which included interviews of staff and children and obtaining written statements from Staff. FR states that child is still enrolled but has not been attending recently. FR states that no medical care was required for incident and children's Authorized Representatives were notified. LPA interviewed staff involved and facility representative regarding child statement,in the incidents and interviews did not confirm a child's personal rights were violated. A notice of site visit was given and must remain posted for 30 days. Exit interview conducted and report was reviewed with the principle/facility representative Kandis Hamblet.
