What the inspector wrote

Page 2 Per Director, she reviews the face to name sheets which show the number of children with the number of staff in the classroom. Per Director, Staff have taken training regarding how to fill out the face to name transition sheets and she has verbally reminded them about adhering to call for backup if needed since the facility has extra staff that would be able to cover. LPA reviewed the face to name sheets for the month of August which showed that at times there were more than 12 children in one classroom, but it was either during napping hours or it was documented that the children were moved to another classroom within minutes of arrival or another staff stepped into the classroom. Based on the evidence gathered, although the complainant accounts that they were informed that the facility was operating out of ratio, due to a lack in determining specific dates and times this allegation occurred, LPA could not make a determination that the facility operated out of ratio. Although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violations did or did not occur, therefore the allegation is unsubstantiated. No deficiencies were cited today. Exit interview was conducted with Director Ana Fragoso-Tovalin. The Notice of Site Visit (LIC 9213) – must remain posted for 30 days during the hours of operation after each site visit by a licensing representative. Failure to maintain posting as required will result in a civil penalty of $100.00. The Notice of Site Visit (LIC 9213) and the Appeal Rights were given and explained to the Facility Representative.
