Licensing Program Analyst (LPA) Lilia Hernandez conducted an unannounced case management inspection on 05/02/2024 to follow up on incidences that were reported to the department. LPA arrived at the facility at 1:10PM and met with Viken Kazarian, Principal who guided LPA on a tour of the facility. There were 55 children and 16 staff present upon arrival. The incidences that occurred at the facility on 04/09/2024, were reported to the Department on 04/12/2024, via telephone. The facility made the report to the Department within the required 24 hours of occurrence. Information reported to the Department indicated that Child #1 and Child#2 personal rights may or may not have been violated. LPA conducted interviews and obtained rosters and other pertinent information. Additional interviews will need to be conducted to determine if Child #1 and Child #2 personal rights were violated. There were no deficiencies cited during today’s inspection. A notice of site visit was given and must remain posted for 30 days.
