Licensing Program Analyst (LPA) Jeanine Lipsey conducted an unannounced case management inspection on 11/6/25 to follow up on an incident that was reported to the Department. LPA arrived at the facility at 1:30PM and met with Lead Teacher Barry Rubenstein, who guided LPA on a tour of the facility. There were 67 children and 15 staff present upon arrival. The incident that occurred at the facility on 10/28/25, was reported to the Department on 10/29/25, via email. The facility made the report to the Department within the required 24 hours of occurrence. Information reported to the Department indicated that Child #1 C1 hit Child #2 C2 on the head with a small shovel and C2 sustained a cut and contusion on the left side of the forehead. During todays inspection, LPA obtained a copy of the facility roster, emergency card, photos, Notice of first aide form, Cautions regarding Head Injury form and LPA also conducted an interviews with Staff#1 and Staff #2. Due time time constraints, the inspection will be concluded at a later time. A notice of site visit was given and must remain posted for 30 days. Exit interview conducted and report was reviewed with Lead Teacher Barry Rubenstein.
