On 4/29/2026 at 9:20AM, Licensing Program Analyst (LPA) Amelia Morales conducted an unannounced Case Management Visit to this facility. Upon arrival, LPA met with Principal Viken Kazarian who guided LPA on a tour of the facility. LPA explained the purpose of today's visit to follow-up on an incident which occurred on 4/20/2026. There were 82 children being supervised by 19 staff. The incident that occurred on 04/20/2026, was reported to the Department on 04/20/2026, via email. The facility reported the Unusual Incident to the Department within the required 24 hours of occurrence. The written report has been received to the Regional Office. The incident which occurred, Child #1 received an injury while in care. Based on interviews conducted Child #2 pushed open a restroom stall door, while Child #1 was inside. The door made contact with Child #1, resulting in a cheek injury. There was active supervision. First Aid was administered, and parents were notified. Child #1 returned to facility next day on 4/21/2026. LPA collected children's roster, conducted interviews with staff and children, and obtained other pertinent information. There were no deficiencies cited during today’s inspection. A notice of site visit was given and must remain posted for 30 days. Exit interview conducted and report was reviewed with Principal Viken Kazarian.
