What the inspector wrote

On 04/24/2026, Licensing Program Analysts (LPAs) Elicia Calvillo and Nathanael John Mooberry conducted an unannounced Case Management -Incident inspection to follow up on an incident that was reported to the department. LPA arrived at the facility at 08:00AM and met with Jennifer Maddocks, Principal, who guided LPA on a tour of the facility. There were 56 children and 7 staff present upon arrival. The incident that occurred on 04/08/2026, was reported to the Department on 04/09/2026, via telephone. The facility reported the Unusual Incident to the Department within the required 24 hours of occurrence. Information reported to the Department indicated that Child #1 was injured by Child #2. LPAs conducted interviews and obtained documentation during this visit. Based upon information received from the interviews conducted it was determined that Child #1 was injured by Child #2. The facility provided support for the children and families and has implemented lesson plans and additional support into their classroom activities. There were no deficiencies cited during today’s inspection. The Notice of Site Visit (LIC 9213) – must remain posted for 30 days during the hours of operation after each site visit by a licensing representative. Exit inter view conducted and report was reviewed with Jennifer Maddocks, Principal.
