What the inspector wrote

Licensing Program Analysts (LPAs) Tatiana Bickham and Decondia Ferguson conducted an unannounced case management inspection due to an incident that occurred at the facility. LPAs arrived at the facility at 4:08 PM and met with Patricia Razo, Principal, who guided LPAs on a tour of the facility. There were 79 children in care and 17 staff present upon arrival. The purpose of the visit was to follow up on an incident that was reported to the Department. The facility is unsure of the exact date the incident occurred. The incident was reported to the Department on 11/13/2025 via telephone. Information reported indicated that a parent witnessed a staff member (S1) pull a child (C1) by the arm during nap time, take the child’s stuffed animal, and that the child fell onto the bed. Parent that made the report did not indicate to the facility when the incident occurred. Per interview with S1, S1 does not recall pulling a child by the arm. S1 stated there was an incident involving a child who was guided by the hand to their cot, but S1 denied pulling the child. Per staff interviewed, no staff member witnessed the incident. Staff stated that a parent informed them they observed a staff member pull a child and take a stuffed animal from the child, but no staff interviewed witnessed the event themselves. Based on staff interviews, LPAs were unable to determine whether the incident did or did not occur. Page 1. There were no deficiencies cited during today’s inspection. A Notice of Site Visit was provided and must remain posted for 30 days. An exit interview was conducted, and the report was reviewed with Principal Patricia Razo. Page 2.
