What the inspector wrote

On January 30, 2026, at approximately 9:35 AM, Licensing Program Analyst (LPA), Janet Gil, conducted a case management inspection and met with the director, Patricia Razo. The purpose of the inspection was explained to the director. The facility operates from Haddon Avenue Elementary School. Present during today’s visit included 24 staff members (including the director) and 111 preschool children. The facility operates within its capacity and in accordance with the required ratio of staff to children. All adults present working in the facility have criminal record clearance through the school district. This case management inspection was related to the Unusual Incident Report (UIR) submitted on December 23rd, 2025. LPA discussed the nature of the incident during the inspection. The incident occurred on December 22, 2025, when a child(C1) was reading in the outdoor library area and swung their feet inside a seating area referred to as the "egg" at the same time as another child(C2) placed their head inside the opening of the "egg". C1 hit C2 with their foot causing an injury under C2's left eye in cheek area. First Aid was provided for C2 by the facility and parents were notified. Per director, since the incident, the facility has assigned a staff to supervise outdoor library area. Director also held a staff meeting (1/6/2026) where she reminded staff supervision expectations for outdoor play time and the importance of redirection. Staff also reminded students how to use materials and equipment appropriately. Per director, there has been no more incidents in outdoor library area. LPA inspected the facility indoors and outdoors for health and safety hazards. No deficiencies were cited today under CCR, Title 22, Div. 12, Chapt. 1. A copy of today’s report was given to the director, Patricia Razo. A Notice of Site Visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100. Exit interview conducted and report was reviewed with the director, Patricia Razo.
