What the inspector wrote

On 5/1/2026 Licensing Program Analyst (LPA) Suzette Ornelas conducted an unannounced case management inspection at the address mentioned above for the purpose of obtaining further information for the unusual incident report reported on 4/17/2026. Upon arrival LPA was greeted by Principal, PATRICIA RAZO and observed 101 children and 14 staff. The facility operates from Haddon Avenue Elementary School. According to the unusual incident report reported on 4/17/2026, on 4/16/2026 Child 1 (C1) stated to teacher that classmate, Child 2 (C2) touched them inappropriately. RP stated that C1 went home and told parents, whom contacted principal. During todays inspection, LPA Ornelas inspected the restroom where the incident took place, conducted interviews and obtained documentation in the form of children and staff sign in/out sheets for 4/16/2026. LPA Ornelas observed restroom utilized to be an open space restroom shared by the attached classroom. According to staff interviewed, during the time of the incident, there were two staff present in the restroom. Staff 2 (S2) and Staff 3 (S3), S2 stated that they were in the restroom assisting 2 other children when the incident occurred and did not observe the incident take place. C1 was not present ant the facility during todays visit. Staff 1 (S1) stated that there is currently a safety plan in place that includes C1 and C2 using separate restrooms at separate times along with reviewing restroom and reporting protocols. No further incidents have taken place. At this time no deficiencies are being cited. A copy of this report and a notice of site visit. The Notice of Site Visit (LIC 9213) – must remain posted for 30 days during the hours of operation after each site visit by a licensing representative.
