What the inspector wrote

On October 21, 2025, Licensing Program Analysts (LPA) Priscilla Ochoa conducted an unannounced case management inspection for the above facility. LPA was greeted by Principal, Ani Nicholas, who guided LPA on a tour of the facility. The purpose of this inspection is to follow up on a self reported incident that occurred at the facility on 09/18/2025 . This is a Preschool Program that operates Monday \342\200\223 Friday from 7:00 am \342\200\223 5:30 pm. There are currently 91 children enrolled, and LPA observed 55 preschool children with 16 staff members. Brief summary of incident: Child #1 (C1) alleged that C1 was pinched and hit on the head by Staff 5 (S5) while in care.. In course of the investigation, LPA interviewed Staff #1 (S1) \342\200\223 Staff #5 and Child 2(C2) \342\200\223 Child 5 (C5). LPA attempted to interview Child 1 (C1) via telephone, but was unsuccessful. LPA also interviewed Parent 1(P1). Based on interviews and observations conducted there was no corroborating information to determine that S5 pinched and hit C1 in the head while in care. All reporting requirements were met. There are no deficiencies being cited at this time. A notice of site visit was given and must remain posted for 30 days during facility\342\200\231s hours of operation. Failure to comply with posting requirements shall result in an immediate civil penalty of $100. An exit interview was conducted, and a copy of this report was provided to Principal, Ani Nicholas.
