What the inspector wrote

Licensing Program Analyst (LPA) Lilia Hernandez conducted an unannounced case management inspection due to a self-reported incident that occurred at the facility. LPA arrived at the facility at 2:10AM and met with Goar Gegeyan, Principal, who guided LPA on a tour of the facility. There were 36 children in care and 20 staff present upon arrival . An incident was reported to the facility on 12/04/2024. The facility reported the incident to the Department on 12/04/2024, via telephone. The facility reported the Unusual Incident to the Department within the required 24 hours of occurrence. Information reported to the Department indicated that an unidentified individual may or may not have violated the personal rights of Child #1. LPA conducted an interview with Principal. Per Principal, the incident was reported to the facility by the Parent of Child #1. Principal disclosed that attempts were made to interview Child#1. Child #1 did not provide any information regarding the incident Child #1 reported to their parent. Principal also disclosed that Child#1 is no longer in care. LPA attempted to reach Parent of Child #1 and was unable to reach parent to conducted an interview. Based upon information LPA was able to obtain, it was determined that there was not enough information to determine if the personal rights of child #1 were violated. There were no deficiencies cited during today\342\200\231s inspection. The Notice of Site Visit was given and must remain posted for 30 days during the hours of operation after each site visit by a licensing representative. Exit interview was conducted and report was reviewed with Goar Gegeyan, Principal.
