What the inspector wrote

Licensing Program Analyst (LPA) Lilia Hernandez conducted an unannounced case management inspection on 06/13/2025 due to a self reported incident that occurred at the facility. LPA arrived at the facility at 2:45 AM and met with Veronica Mentaze, Head Teacher, who guided LPA on a tour of the facility. There were 35 children and 15 staff present upon arrival. LPA was later met by Sheila Taylor-Hardy, Principal. The incident that occurred on 05/20/2025, and was reported to the Department on 05/21/2025, via email. The facility reported the Unusual Incident to the Department within the required 24 hours of occurrence. Information reported to the Department indicated that Staff #1 may have violated the personal rights of Child #1 while in care. During today's visit, LPA conducted an interview with Staff #3, obtained a copy of the facility roster, and other pertinent information. Based on disclosures LPA received today, additional information is needed to determined if the personal rights of Child #1 were violated. There were no deficiencies cited during today\342\200\231s inspection. The Notice of Site Visit was given and must remain posted for 30 days during the hours of operation after each site visit by a licensing representative. Exit interview was conducted and report was reviewed with Sheila Taylor-Hardy, Principal.
