What the inspector wrote

Licensing Program Analyst (LPA) Lilia Hernandez conducted an unannounced case management inspection due to an incident that occurred on 05/16/2024. LPA arrived at the facility at 2:18PM and met with Militza Maldonado Garcia, Facility Representative, who guided LPA on a tour of the facility. There were 40 children and 16 staff present upon arrival. LPA was later met by Jennifer Movsessian, Principal. The purpose of the visit was to follow-up on an incident that was reported to the department. The incident that occurred on 05/16/2024, was reported to the Department on 05/16/2024, via email. The facility reported the Unusual Incident to the Department within the required 24 hours of occurrence. Information reported to the Department indicated that Staff#1 observed Child#1 outdoors playing on the climbing structure unsupervised. LPA conducted interviews and obtained documentation during this visit. Disclosures made indicated that there was 4 staff members supervising 10 children outdoors in the play yard. Based on information LPA received from the interviews, there were no deficiencies cited during today’s inspection. The Notice of Site Visit was given and must remain posted for 30 days during the hours of operation after each site visit by a licensing representative. Exit interview was conducted and report was reviewed with Jennifer Movsessian, Principal.
