What the inspector wrote

On 4/20/2026, Licensing Program Analysts (LPAs) Amelia Morales and Christina Nunez conducted an unannounced Case Management Visit to this facility. Upon arrival, LPA met with Principal Jennifer Movessian who guided LPA on a tour of the facility. LPA explained the purpose of today's visit to follow-up on an incident which occurred on 4/14/2026. Census: There were 73 children being supervised by 16 staff. Room 2 A: 18 children, 5 staff Room 3 A: 16 children, 4 staff Room 2 B: 19 children, 4 staff Room 3 B: 20 children, 3 staff The incident that occurred on 04/14/2026, was reported to the Department on 04/14/2026, via email. The facility reported the Unusual Incident to the Department within the required 24 hours of occurrence. The written report has been received to the Regional Office. The incident which occurred, indicated that Staff #1 may have violated the personal rights of Child #1 while child was in care. Child #1 was not present at the time of the visit. Page 1 of 2 LPAs collected children's roster, conducted interviews with staff, and obtained other pertinent information. It should be noted that during the time of the visit, the facility lost power at approximately 11:07AM. The power returned at 12:02PM, LPAs advised to call the Regional Office and report the incident. LPAs advised to submit written report, Principal Movsessian was able to provide LPA with the written report. Due to insufficient information, further investigation is needed. A notice of site visit was given and must remain posted for 30 days. Exit interview conducted and report was reviewed with Principal Jennifer Movsessian. Page 2 of 2
