What the inspector wrote

Licensing Program Analyst (LPA) Alicia Mooberry conducted an unannounced Annual Random Inspection on this date. Upon arrival LPA met with Maria Bautista, Licensee. LPA informed of the purpose of inspection and provided form LIC 126, Inspection Entrance Checklist. The Licensee provided tour of facility. LPA inspected rooms/areas on the facility sketch in which child-care services are provided and to which children have access. Per licensee the current hours of care provided are Monday-Friday 6:00am - 6:00pm . There were 3 children present (ages 2-4 years). Also present was Adult 1, assistant. All adults present have obtained the required background clearance and are associated to the facility. Individuals residing in the home were discussed and noted. This is a single-story home which consists of three bedrooms and two bathrooms, kitchen, living room, and playroom. There is a fireplace in the home that is barricaded by a gate and LPA observed to be locked. Areas used by the children include: Living Room (located at entry of the home), Bathroom 1 (located in hallway by Living Room and next to 2 bedrooms), Kitchen/Dining Room, and playroom (in the rear of the home, located through Bedroom 1). Areas off limits to children and parents include: Two bedrooms by living room (observed to be locked), Bathroom 2 located inside bedroom, covered and gated patio in back yard, garage/laundry room, storage shed, dog run and front yard. LPA observed that there is a gate separating living room from dining room & kitchen. Per Licensee, Bedroom 1 is only accessible as a walkway from kitchen to playroom LPA discussed making sure children are supervised as they walk through Bedroom 1 to access Playroom. LPA observed no hazards in Bedroom 1. Page 1 – Report Continues LPA observed the facility license, Publication (PUB) 394- Notification of Parent Rights and Licensing Form (LIC) 9148- Earthquake Preparedness form posted in playroom. Licensee was reminded to post the required posted documentation in a location visible to parent/guardians of children in care. LPA reviewed Children’s Roster form LIC 9040 and Emergency Disaster Plan (LIC 610A) The home is observed to be clean and orderly. Smoke and carbon monoxide detectors were tested and are operable. Fire extinguishers (in living room and kitchen) indicated fully charged and were serviced on 2/11/26. The home maintains telephone service via cell phone/LAN line. LPA observed that cleaning compounds are inaccessible to children in care in locked cabinets in the kitchen, bathroom and laundry room. The bathroom that children use observed to be clean and free of hazards. Licensee states that poisons are stored in the locked garage not accessible to children. Per Licensee there are no firearms or weapons stored in the home. The facility does not have a pool or similar bodies of water. Per licensee, there is 1 large bread dog that is kept in inaccessible room during daycare hours. Per Licensee, area for sick children waiting to be picked up is in LOCATION, supervised and away from the other children. There are toys and other age-appropriate material available inside the home and in the outdoor play are. Outdoor play area is in the back yard and observed to be enclosed. LPA observed that the outdoor yard has a two-level wooded and plastic play structure/, which is anchored to the ground with cushioned material under structure. Three (3) Children’s records were reviewed for (LIC) 282- Affidavit Regarding Liability Insurance, Immunization's Records, LIC 700- Identification and Emergency Information, LIC 627- Consent for Medical Treatment, LIC 995A Notification of Parents’ Rights. Staff records were reviewed for approved Pediatric First Aid and CPR certification, LIC-501: Personnel Record, LIC 508-Criminal Record Statement, LIC 9052- Employee Rights, Proof of immunizations against measles, pertussis and influenza or influenza declination, TB clearance or risk assessment, LIC 9108- Statement Acknowledging Requirement to Report Child Abuse and current Mandated Reporter Training Certificate. Licensee was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California. ----Page 2, report continues To improve the quality and value of the new inspection process, a survey may be sent to the email address provided. Licensee was reminded that all adults 18 and over living or working in the home, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated. LPA discussed the safe sleep regulations with licensee and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep as an additional resource. LPA also informed licensee of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment. Incidental Medical Services (IMS) policy was discussed. For IMS information see PIN 22-02-CCP . When any IMS is provided, a Plan for Providing IMS must be submitted to the Department. During the exit interview, the licensee, confirmed that there are no Registered Sex Offenders living in the facility and LPA completed the RSO profile in FAS. Based on the LPA's observations and records review 1 B deficiency is cited during this inspection. A notice of site visit was given and must remain posted for 30 days.
