What the inspector wrote

A copy of this report must be provided to the authorized representative of all currently enrolled children and any newly enrolled child for the following 12 months. The acknowledgement of receipt of Licensing Reports (LIC 9224) shall be signed and kept in each of the children\342\200\231s records. The notice of Site Visit (LIC 9213) must remain posted for 30 days during the hours of operation after each site visit by a licensing representative. Failure to maintain posting as required will results in a civil penalty of 100.00. Exit interview conducted, report and appeals rights were discussed and provided to Director.

  • TEACHER-CHILD RATIO

    (a) There shall be a ratio of one teacher visually observing and supervising no more than 12 children in attendance, except as specified in (b) and (c) below. This requirement was not met as evidenced by: Based on LPA's record review of staff time cards and children sign in/out sheets. 1 teacher was found supervising more than 12 children on two separate days. Facility was operating beyond staffing ratios. This is a type A Deficiency which poses an immediate Health and Safety, and personal rights risk to persons in care.

  • PERSONAL RIGHTS

    (a) The licensee shall ensure that each child is accorded the following personal rights:(3)To be free from corporal or unusual punishment, infliction of pain, humiliation, intimidation, ridicule, coercion, threat, mental abuse or other actions of a punitive nature including but not limited...... This requirement was not met as evidenced by: Based on interviews with parents. Three parents expressed issues and concerns regarding diapering and toileting needs with their child, which poses a potential health, safety, or personal rights to persons in care.
