What the inspector wrote

On 5/7/2025, Licensing Program Analyst (LPA), V. Wheatley conducted an unannounced Annual Inspection for the preschool license and met with Lead Teacher Jung Lee. The director Hyun Sook Hong is out of the country until the end of the month. LPA toured the facility indoors and outdoors. LPA observed a total of 30 children playing and later napping. Days and hours of operation are Monday through Friday 7:30am to 6:00pm. There is no swimming pool or other bodies of water on the premises . There are no firearms or ammunition allowed or stored on the premises. Furniture and equipment are in good condition, free of sharp, loose or pointed parts. Playground equipment is in safe condition, free of sharp, loose or pointed parts. Areas around high climbing equipment, slides have cushioning material to absorb falls. The facility is free of flies, insects and rodents. All toilets and hand washing facilities are in safe and sanitary operating condition. Floors in the facility are clean and safe. LPA observed several electrical outlets uncovered in the classroom for the youngest age room. The outlets were covered during the inspection. LPA observed cleaning solutions, detergents accessible to children under the sink in Green door classroom. The items were removed during the inspection. There is an isolation area for ill children inside of the office. PAGE 2 All kitchen, food preparation and storage areas are clean, free of litter/rubbish and free of rodents/vermin. All food is protected against contamination and any contaminated food is discarded immediately. Solid waste storage containers have tight-fitting covers and are in good repair. Drinking water is available and the children are allowed to use water bottles which are labeled. There are filter water dispensers on the premises. There are fire extinguishers, working carbon monoxide detector. Menus are posted monthly in advance where an authorized representative can view them. LPA inspected the kitchen and food preparation area which is clean and orderly. LPA did not observe any expired foods. Prior to working or volunteering in a licensed child care facility, all individuals subject to a criminal record review have received a criminal record clearance or exemption. Upon notification from the Department, the licensee will comply and act immediately to terminate the employment of, remove from the facility or bar from entering the facility for any person it is deemed necessary while the Department considers granting or denying an exemption. LPA observed three staff members on the premises without fingerprint clearances. See LIC 809D. Capacity and limitations as specified on the license are being maintained. At least one person trained in CPR and Pediatric First Aid is present when children are at the facility or at off site activities. The assistant director CPR/first aid expires on 11/2025 and the director's CPR/first aid expires 4/2026. The Mandated Reporter Training for most employees have expired. The person who signs the child in/out of the facility shall use their full legal signature and record the time of day. There is a ratio of 1 teacher supervising no more than 12 children in care. LPA reviewed a sample of children\342\200\231s files and observed files were complete with contact information for authorized representative and or relatives or others who can assume responsibility for the child, and medical assessment. LPA reviewed a sample of staff files and observed three staff without files and without health screening, immunization records for influenza, pertussis and measles and current documentation. Staff records do not reflect the two staff (piano and art teacher) have qualifications. PAGE 3 LPA is concerned about staff supervising the children. LPA observed the art teacher and piano teacher supervising children without units or a file. See LIC 809D. Incidental Medical Services (IMS) is currently not being provided to any children. Director is aware that an IMS plan is required to be submitted to the licensing office if they provide any of these services. LPA and Director discussed the Community Care Licensing website www.ccld.ca.gov which will provide access to Provider Information Notices (PINs), Quarterly Updates, Mandated Reporter Training, Lead Poisoning Facts, Forms and Regulations. Per Title 22, Division 12, Chapter 3, of the California Code of Regulations there are deficiencies cited. An exit was conducted, a copy of this report was read and will be provided. This report shall be made available to the public upon request. The parents will be provided the LIC 9224 to sign. LIC 9213 Notice of Site Visit was provided and required to be posted for 30 days .

    CRIMINAL RECORD CLEARANCE

    Based on LPA Wheatley's observation, interview and record review, the licensee did not comply with the section cited above in that three staff members (cook, piano teacher, art teacher) are not fingerprinted or live scanned. LPA called the El Segundo office and verified that the staff do not have a fingerprint clearance. The cook is around the children daily. The piano teacher was supervising children alone today. The art teacher is here once a week however she teaches the children piano lessons without a fingerprint clearance. This poses an immediate health, safety or personal rights risk to persons in care.

  • TEACHER AIDE QUALIFICATIONS AND DUTIES

    Based on LPA's observation and interview with lead teacher, the licensee did not comply with the section cited above in that two staff members (piano teacher and art teacher) were supervising children without education/units. These staff members are to work under the direct supervision of a teacher. The children were awake and not napping when this was observed. This poses/posed a potential health, safety or personal rights risk to persons in care.

    Based on LPA's observation and record review, the licensee did not comply with the section cited above in that three employees do not have a TB Test on file. This poses/posed a potential health, safety or personal rights risk to persons in care.

    Based on LPA's observation and record review, the licensee did not comply with the section cited above in that 2 staff do not have a health screening on file. LPA observed the two staff supervising children. This poses/posed a potential health, safety or personal rights risk to persons in care.

LPA observed the certificates expired. This poses/posed a potential health, safety or personal rights risk to persons in care.
