What the inspector wrote

On 05/20/2026, Licensing Program Analysts (LPAs) Cristina Castellanos and Judy Laureano conducted an unannounced case management inspection to follow up on a self-reported Unusual Incident submitted by the facility on 05/04/2026. Upon arrival, LPAs met with Interim Director Goretti Salcido and explained the purpose of the visit. LPAs observed twenty-four (24) children in care and five (5) fingerprint-cleared staff present. On 05/04/2026, Officer of the Day LPA L. Clayton received a telephone call from Child & Family Counselor Elizabeth Medellin regarding an Unusual Incident Report (LIC 624) involving Child 1 (C1) . According to the report, Parent 1 (P1) signed out her children at 4:17 p.m. and proceeded to the preschool yard to locate C1 and Child 2 (C2) . The infant sibling ( C3 ) was with the parent at the time. The report states that C1 was hiding from P1. C1 then came inside the classroom passed P1 and ran out the door leading outside the building. P1 was notified by Teacher Goretti that he ran outside, P1 ran after him about 40 seconds later. As they returned, P1 began yelling towards Teacher Goretti. Per RP, P1 was asked to leave the center. During today’s inspection, LPAs Castellanos and Laureano requested and reviewed the following documents: § Children’s roster § Children’s files § Staff roster § UIRs (LIC 624) and Ouchie/Incident Reports for April & May 2026 Continue § Written employee statements § Relevant facility documentation § Camera footage Additional activities conducted: § Staff interview Based on interviews, document review, and camera footage, LPAs determined that on 05/01/2026 , C1 left the back outdoor play area, entered the Young Preschool classroom, opened the classroom’s main door, exited into the main lobby, opened the facility’s main entrance, and ran toward an open corner gate, ultimately approaching a parked vehicle on the sidewalk. Interim Director Salcido stated that the corner gate lock is broken and that the facility is not responsible for repairs to the complex. This constitutes a Type A violation due to the immediate risk to the health and safety of children in care. Additionally, interviews and documentation confirm that children were present during the verbal altercation between P1 and staff. This constitutes a Type B violation for failure to ensure a safe and appropriate environment. Pursuant to Title 22, Division 12, Chapter 1, two deficiencies were cited and documented on the LIC 809D (Type A & Type B) . Upon receipt of this report, the Licensee must post the Notice of Site Visit and any licensing report documenting a Type A deficiency for 30 consecutive days. Failure to maintain the required posting will result in an immediate $100 civil penalty. A copy of this report must be provided to all currently enrolled parents/guardians by the next business day or upon their return. For the next 12 months, a copy must also be provided to parents/guardians of newly enrolled children. The Acknowledgment of Receipt of Licensing Reports (LIC 9224) must be placed in each child’s file upon receipt. A copy of the LIC 9224 was provided during today’s visit. An exit interview was conducted, and the Plan of Corrections was reviewed with Interim Director Salcido. A copy of this report, the Notice of Site Visit, and the Appeal Rights were provided.
