What the inspector wrote

On 05/20/2026 , Licensing Program Analysts ( LPA ) Cristina Castellanos and Judy Laureano conducted an unannounced case management – incident inspection to follow up on a self-reported Unusual Incident submitted by the facility on 0 5 / 14 / 2026 . Upon arrival, LPAs Castellanos and Laureano met with Interim Director Goretti Salcido and explained the purpose of the visit. LPAs Castellanos and Laureano observed twenty-four ( 24 ) children in care and five ( 5 ) fingerprinted staff members . On 05/15/2026 , Officer of the Day LPA C. Castellanos received a telephone call from Child & Family Counselor Elizabeth Medellin regarding an Unusual Incident Report (UIR) involving Child 1 (C1). According to the report on 05/14/2026 Child 1 (C1) sustained an injury to the head resulting in a bump with a bleeding cut. RP further stated that no one actually witnessed the incident. During today’s inspection, LPAs Castellanos and Laureano requested and reviewed the following documents: § Children’s roster § Children’s files § Staff roster § UIRs (LIC 624) and Ouchie/Incident Reports for April & May 2026 § Child Accident Report § Camera footage Continue Based documentation review and camera footage, LPAs determined that on 05/14/2026 , C1 sustained an injury to the head due to a lapse of supervision. This constitutes a Type A violation due to the immediate risk to the health and safety of children in care. Upon receipt of this report, the Licensee must post the Notice of Site Visit and any licensing report documenting a Type A deficiency for 30 consecutive days. Failure to maintain the required posting will result in an immediate $100 civil penalty. A copy of this report must be provided to all currently enrolled parents/guardians by the next business day or upon their return. For the next 12 months, a copy must also be provided to parents/guardians of newly enrolled children. The Acknowledgment of Receipt of Licensing Reports (LIC 9224) must be placed in each child’s file upon receipt. A copy of the LIC 9224 was provided during today’s visit. An exit interview was conducted, and the Plan of Correction was reviewed with Interim Director Goretti Salcido. A copy of this report, the Notice of Site Visit, and the Appeal Rights were provided.
