What the inspector wrote

On 5/16/23 Licensing Program Analyst (LPA) Jeanette Estrada conducted an unannounced 1 year Required Annual Inspection. LPA met with Site Supervisor Fernando Perez-Cornejo who provided a tour of the facility. LPA provided the Facility Entrance Checklist to the Site Supervisor. The hours of operation for this program are Monday - Friday 6:30AM to 5:30PM. The facility has 4 classrooms where care is provided. Upon arrival there were 9 children and 2 teachers in Classroom Level 1, 7 children and 2 teachers in Classroom Level 2, 5 children and 2 teachers in Early Head Start (EHS) 3 and 4 children and 2 teachers in EHS 4. Teacher child ratios were observed to be in compliance and staff names recorded. The following was observed during the tour of the facility: All documents required to be posted were observed. All areas used by children were inspected. All classrooms inspected for cleanliness and good repair. The floors were clean and lighting was in operable condition. There were age appropriate toys and equipment. Each child has their own cubby/storage area. Children nap in the classrooms and there are napping cots available for them to use. Sheets are sent home weekly to be cleaned. There are various door alarms located throughout the facility to alert staff. Water is available in the classrooms via a portable water dispensers and disposable cups. The facility tested for lead in water on 10/14/22 and no exceedance levels were identified. There is one restroom available for children with 5 toilets and 5 sinks. There is also a diaper changing area with a sink at an arms length reach. Potty training is available when needed. The restroom and diaper changing area were observed to be clean and in operable condition. The carbon monoxide and smoke detectors in the facility were observed to be in operable condition. LPA observed a Fire extinguisher which indicated fully charged and was last serviced in April 2023. Last disaster drill was performed in February 2023. The facility provides a breakfast, lunch and an afternoon snack. Menus are posted weekly. Meals are prepared at this facility. The kitchen/ food preparation area is inaccessible to children in care. LPA inspected the food preparation area and observed it to be clean and in operable condition. All food was observed to be in good quality. LPA toured the outdoor play area. The outdoor play area is separated into to yards: one for the Level 1 and 2 classroom and one for the EHS classrooms. Outdoor areas were observed to have age-appropriate toys and material for children, free of loose, sharp, and/or pointed parts. Page 2 of report LPA observed required cushioning under climbing structure to absorb fall. Shade was observed in the outdoor area via trees. Outdoor drinking water is provided. There are no bodies of water on the premises. The isolation area is located in the Site Supervisor's office. Ill children may use the children's restroom which is then sanitized after use. The facility administers prescription medication. Medication is stored in the Site Supervisor's office. Per Site Supervisor, none of the children enrolled require medication at this time. Cleaning supplies were observed to be inaccessible in the classrooms. Children's sign in sheets were reviewed. LPA observed the children present today were signed in. Children's records reviewed contained the required forms. Staff records were reviewed. There is at least one staff present with a valid pediatric CPR/first aid certification during this visit. 1 out of 10 staff records reviewed did not contain LIC 503 and the Mandated reporter training for child care providers certificate. All other staff records contained the required items. Director was reminded that all adults 18 and over, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated. Incidental Medical Services (IMS) policy was discussed. When any IMS is provided, an updated Plan of Operation that includes IMS must be submitted to the Department.
