What the inspector wrote

Licensing Program Analyst (LPA) T. Tran conducted an unannounced Case Management Incident visit at MAOF Center Preschool to follow up on a self-reported incident that occurred on 07/31/2025. The Monterey Park Southwest Office received the writing report on 08/01/2025. LPA met with Supervisor, Aracely Garcia then we toured the facility. LPA observed proper care and supervision. LPA completed child and staff’s files review. LPA obtained child's documents and personnel report. LPA conducted interviews with children, staff, and others. According to the record reviewed and interviews conducted. On the day of the incident, there were 10 children with two staff. The incident took place during morning recess, children were playing soccer, C1 tripped and hit the head on the bench. C1 sustained an open cut on the right eyebrow and medical attention required. Staff immediately provided first aid and managed to stop the bleeding. Staff contacted the child’s parents in a timely manner. Child was taken to the doctor and superficial glue applied. The child returned to school the next day without any restrictions. At this time, based on the available information, it does not appear this incident was the result of a Title 22 violation of lack of care and supervision. No further action is required for this incident. No deficiency was cited at this time. A notice of site visit was given and must remain posted for 30 days. Exit interview conducted and the report was reviewed with the facility representative, Aracely Garcia.
