What the inspector wrote

On 2/2/2026 Licensing Program Analyst (LPA) Judy Laureano conducted an unannounced 3 Year Required Inspection at the above-mentioned facility. LPA was greeted by Lead Teacher Ms. Gaxiola and Miss Alfaro who lead LPA on a tour. Facility director was contacted and arrived shortly. At approximately 9:32 a.m. Program Director Nellie Rios- Parra followed by Site Supervisor Christina Hector arrived. LPA observed all 5 classrooms, currently the program is only using one classroom; classroom 2. The following was observed during the initial inspection: Classroom 1, 3,4, and 5- not being used. Classroom 2- classroom currently being used for program. LPA observed 13 children and 2 staff members providing care and supervision. Facility operates on a Lennox School district campus. It is physically on the Middle School site. Days and hours of operation are Monday through Friday 7:50 a.m. .to 10: 50 am and 12 p.m. to 3:00 p.m. Program offer an AM and PM session. Facility is licensed to serve 105 preschool children ages 2 years through entry to First Grade. Facility is open from 7:30 am to 4:00 p.m. Medications are made inaccessible and stored in a locked classroom cabinet. Currently facility has one child that requires medication. If medication needs to be refrigerated, medication will be stored in director's office. All necessary paperwork that needs to be included with medication is stored in the classroom with medication and director's office. Teachers do the day to day cleaning in the classroom. Disinfectants, cleaning solutions and other hazardous items are made inaccessible in locked classroom cabinets. Cabinets were observed to have safety latches making the content inaccessible to the children in care. School district maintenance comes daily after school and mid-day to disinfect classrooms. Page 2 Capacity and limitations as specified on the license are being maintained. Currently only 1 out of the 5 classroom are being used. Furniture and equipment are in good condition, free of sharp, loose or pointed parts. Playground equipment is in safe condition, free of sharp, loose or pointed parts. The surface of the outdoor activity space is maintained in a safe condition and is free of hazards. All toilets and handwashing facilities are in safe and sanitary operating condition. Floors in the facility are clean and safe. Facility provides meals through the school district; Buford Elementary school provides meals for the facility. Menu is posted in classroom's parent board. Parents also receive menu via Class Dojo online application and school district website. All food is protected against contamination and any contaminated food is discarded immediately. Preschool program coordinates with school cafeteria staff to ensure that all dietary restriction are being adhere too. Drinking water is available both indoors and outdoors. Children bring in their own refillable water container and school will provide water bottles when children forget to bring in their water or run out of water. Parent board was observed with all necessary posting in preschool classroom. The facility is free of flies, insects and rodents. There is no swimming pool or other bodies of water on the premises. T here are no firearms or ammunition allowed or stored on the premises. Working fire extinguishers and carbon monoxide and smoke detector were observed in the space. All electrical outlets were observed to be covered. At least one person trained in CPR and Pediatric First Aid is present when children are at the facility or at offsite activities. Pediatric CPR and Pediatric First Aid training has been completed by all staff. Director\342\200\231s Pediatric CPR and Pediatric First Aid training was taken on 8/7/2025. LPA discussed the importance of making sure all Pediatric CPR and Pediatric First Aid training is EMSA approved. LPA reviewed 5 staff files and observed files to be complete. LPA reviewed all other necessary requirements such as Mandated Reporter Training certificate, educational qualifications and Health Screening. Page 3 LPA reviewed a sample of children\342\200\231s files and observed 5 out of 5 files complete with the all the necessary Community Care Licensing (LIC) forms. The name of the child care center director or fully-qualified teacher(s) designated to act in the director\342\200\231s absence has been reported to the Department. The person who signs the child in/out of the facility shall use their full legal signature and record the time of day. Facility uses an electronic sign in/out system; Care Connect. LPA discussed the importance of ensuring the person who brings the child to , and removes the child from, the center shall sign in the child in/out. Director confirmed that hard copies of sign in/out sheets are available for LPA\342\200\231s review. All children are under supervision, including visual supervision, of a teacher at all times. Site director was reminded that all adults 18 and over, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated. Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Child Care Centers (CCCs) constructed before January 1, 2010, to test their water (used for drinking and food preparation) for lead contamination before January 1, 2023, and then every 5-years after the date of the first test as specified in Health and Safety Code section 1597.16. For child care center licenses issued after July 1, 2022, the licensee shall test their water for lead within 180 days of licensure pursuant to Written Directives section 101700 (PIN 21-21.1-CCP.). LPA verified that the lead testing was completed in accordance to the Written Directives (Interim Licensing Standards) outlined in PIN 21-21.1-CCP. Page 4 This facility provides Incidental Medical Services \342\200\223 IMS.
