What the inspector wrote

Licensing Program Analysts (LPAs) Alicia Mooberry and Jonnisha Culbert conducted an unannounced case management inspection to the above facility. Upon arrival LPAs met with Cindy Banuelos, Assistant. LPAs observed seven (7) children including one (1) infant. Also present was Arturo Monteon, Licensee's spouse . Licensee arrived 3:20pm with additional children in care. LPAs toured the facility. The purpose of today's inspection was to ensure the health and safety of children in care and the discuss deficiencies cited on 2/22/23. During this inspection LPAs observed that the care is conducted in the living room. The attached patio was observed to be closed and no children were observed in off limits room. Licensee was reminded to ensure the off limit areas are kept closed and inaccessible to children in care. LPAs reviewed staff and children's files and observed that Cindy Banuelos, Assistant, has not completed Mandated Reporter Training and does not have proof of vaccination against Measles and Pertussis, this poses a potential risk to the health and safety of children in care. Per Licensee and assistant they are requesting a copy from the doctor. Two B deficiencies cited during today's inspection. Licensee was reminded that deficiencies need to be corrected to ensure the health and safety of children in care, failure to correct can result in administration of civil penalties. The Notice of Site Visit (LIC 9213) \342\200\223 must remain posted for 30 days during the hours of operation after each site visit made by a licensing representative. Failure to maintain posting as required will result in a civil penalty of $100.00. Exit interview was conducted with Maria Monteon, Licensee, including, but not limited to Appeal Procedures, Site Visit and Initial Appeal Rights.

    (1) Commencing September 1, 2016, a person shall not be employed or volunteer at a family day care home if he or she has not been immunized against influenza, pertussis, and measles.This requirement is not met as evidenced by: Based on record review, the licensee did not comply with the section cited above in Staff 1 did not have proof of immunizations which poses a potential health, safety or personal rights risk to persons in care.
