What the inspector wrote

On 6/24/25 at 1:10 p.m., Licensing program Analysts (LPAs) Ana Rodriguez, Hanna Cha, and Licensing Program Manager (LPM) Mariela Ramon arrived at the facility for the purpose of a Case Management visit and met with Licensee Maria Gladys Zaldana who granted access. LPAs and LPM observed Licensee, 1 assistant, and 3 preschool and 3 school aged children. All adults cleared as verified in Guardian. During the Case Management Inspection, LPM and LPAs observed the following deficiency: At 10:30 am upon entry to the facility, LPAs and LPM observed in the outdoor play area 2 broken plastic teeter toters/rockers with sharp edges, a metal wire wrapped around the door and windows of play house, and a broken plastic storage container and shelf. The facility is non-compliant with the following Title 22 Regulation - 102417 (d) Operation of a Family Child Care Home : The home shall provide safe toys, play equipment and materials. During this visit, licensee removed the 2 broken plastic teeter toters rockers and the metal wire around the door and windows of the play house. Please see Evaluation Report LIC809D for deficiency cited. Appeal rights was provided to the Licensee. A notice of site visit was given to licensee An exit interview conducted, and report was reviewed with the Licensee Maria Gladys Zaldana.
