What the inspector wrote

Licensing Program Analysts (LPA) Brittanee Cleveland conducted an unannounced case management inspection on 06/03/2026 due to a self-reported incident that occurred at the facility. LPAs arrived to the facility at 12:01 PM and met with Carrie Zuckerman, Owner and Co-Director, who guided LPA on tour of facility. There were 100 children in care and 31 staff present upon arrival. At the time of visit, Director stated they wanted to report an unusual incident report that occurred. LPA was given the Unusual Incident Report form (LIC 624). LPA advised any additional documents can be sent via e-mail. LPA conducted an interview with Co-Director, whom stated that while a child was playing a pebble hit them in their cheek. Child was observed and given first aid when incident occurred. Child had a red mark on cheek after incident. Co-Director states the child returned to school and appeared to be feeling normal. LPA advises staff to continue to observe child, in case there are any changes to injury. If any changes occur, staff shall report to the LPA or department with an update. Exit interview was conducted with Carrie Zuckerman, Owner and Co-Director. Notice of Site Visit and Appeals Rights provided.
