What the inspector wrote

Licensing Program Analyst (LPA) Tatiana Bickham conducted an unannounced Plan of Correction (POC) Visit on 10/27/2025 at 1:25 PM. Upon arrival, LPA disclosed the purpose of the inspection and met with Licensee Gevorg Terzyan, who guided the LPA on a tour of the facility. There was 1 infant present during today\342\200\231s inspection. LPA also observed 1 staff present at the time of this inspection. During the Annual Required Inspection conducted on 10/03/25, the following deficiencies were issued: Type B - 102417(g)(9)(A)1- Fire and Disaster Drills. Type B- 102425(j)(2)(D)(c)- 15 minute safe sleep documentation. Type B - 1596.8662(b)(1)- Mandated Reporter Training. Type B - 102421(b)- Incomplete children files. Technical Violation- 102416(c) - Pediatric CPR/First Aid. 102417(g)(9)(A)1 - LPA observed fire and disaster drill form with drill conducted on 10/10/25. 102425(j)(2)(D)(c)- LPA observed 15-minute safe sleep documentation for infant in care. 1596.8662(b)(1)- LPA observed completed Mandated Reporter Training certificate for Licensee and Licensee's assistant with expiration date of 10/2027. 102421(b)- LPA observed completed files for all 7 children enrolled. 102416(c) - LPA observed completed Pediatric CPR/ First Aid certificate for Licensee and Licensee's assistant with expiration date of 10/2027. The deficiencies noted above have all been cleared and no further action is needed. Page 1. LPA will submit application for a large family child care with an capacity of 14 for approval. The Notice of Site Visit was provided and must remain posted for 30 days during the hours of operation after each site visit by a licensing representative. Failure to maintain posting as required will result in a civil penalty of $100.00. Exit interview was conducted with Gevorg Terzyan and Appeals Rights provided. Page 2.
