What the inspector wrote

On 7/30/2025, Licensing Program Analyst (LPA) Suzette Ornelas conducted an announced Annual/Capacity Increase Inspection at 13603 HART STREET VAN NUYS CA 91405. There is another address located on the property leased by other tenants (13601 Hart Street Van Nuys Ca 91405). The purpose of the meeting was to ensure that health, safety and personal rights as required by Title 22 Regulations governing California Family Child Care homes will be met by the Licensee, AILEEN ASTVASATURIAN. The licensee is applying for an increase of capacity; Large Family Child Care Home license for a maximum capacity of 14 children-ages 0 to 13 years of age. The Fire clearance was granted on 7/10/2025. The family child care home will operate Monday through Sunday 7 a.m. to 11:30 p.m. Licensee guided LPA on a tour of the inside and outside of the home. LPA observed 1 fingerprint cleared adults and 0 children in care. Current facility sketch reviewed and Licensee confirmed that the entry of the day care is through the front door of the home. All areas identified on the facility sketch were inspected, including but not limited to, off limit areas. This is a single story home that consists of 3 bedrooms, 3 bathrooms, dining room, living room, and kitchen. Children will enter through the front of the home. Kitchen, Dining Room and Living Room are on limits only when children enter/exit. Bedroom 3/Kids Room is on limits and accessible to children in care. Bedroom 3/Kids Room has a door that leads the kitchen/dining room and a door that leads to a hallway. Bathroom 3 located within Bedroom 3 is on limits and accessible to children in care. Doors have safety door knob covers which ensures children do not have access to the off limit areas. All other areas of the home (Bedroom 1, Bathroom 1, Bedroom 2, Bathroom 2 are off limits to children in care and made inaccessible by closed doors, safety door knob covers and supervision. The back yard has been sectioned off. Children have access to the play yard that is accessible from the left side of the home. The play yard has a gate over 5 feet high that makes the rest of the back yard inaccessible to children in care. The back yard has a fishpond. The fishpond measures 8 ft 10 inch in diameter and 2 ft 5 inch deep. The fishpond has been fenced and is inaccessible to children in care. The day care room is spacious enough for 14 children which also provides access to the on limits bathroom. The home was inspected inside and out for safety, comfort, cleanliness, telephone service, heating and ventilation, inaccessibility to poisons, detergents, cleaning compounds, medicines, and hazardous items that can pose a danger to children. Per LIS the facility annual fees are current. The facility roster was observed, and current. There are age appropriate toys and napping equipment on the premises. The required fire extinguisher (2A-10BC) was serviced on 9/24/2024. Carbon monoxide detectors and smoke detectors are in operable condition. Licensee has posted as required the License, and all other required postings in a visible location. There are no firearms or ammunition on the premises. There is no swimming pool on the premises. Fireplace located in living room has been screened and made inaccessible to children in care. There are currently infants in care. Licensee ensures that children in care are supervised at all times and is aware children shall not be left in parked vehicles. Car seats are used for transportation purposes only and are not used for sleeping children. The outdoor play area in the backyard is fenced and there are no hazards to children present. LPA discussed the safe sleep regulations with licensee and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep as an additional resource. LPA also informed licensee of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment. Licensee\342\200\231s pediatric CPR/First Aid is current. A review of records indicates that all employees and/or volunteers have immunization records on file for influenza, pertussis and measles. Licensees have completed the Mandated Reporter Training within 2 years. All adults who reside or work in the home have a criminal record clearance or exemption. There are no excluded individuals present at this home. Incidental Medical Services (IMS) are not currently being provided. Licensee is aware that an IMS plan is required to be submitted to the licensing office if they provide any of these services. Licensee was reminded that all adults 18 and over living or working in the home, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated. Licensee was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California. During the exit interview, the Licensee, confirmed that there are no Registered Sex Offenders living in the facility and LPA completed the RSO profile in FAS. LPA and Licensee discussed the Community Care Licensing website www.ccld.ca.gov which will provide access to Provider Information Notices (PINs), Quarterly Updates, COVID-19 Information and Resources, Mandated Reporter Training, Safe Sleep in Child Care, Lead Poisoning Facts, Forms and Regulations. This report shall be made available to the public upon request. A notice of site visit was given and must remain posted for 30 days. Exit interview conducted and report was reviewed with the licensee, AILEEN ASTVASATURIAN. Final license determination will be made upon review by the Licensing Program Manager
