What the inspector wrote

Licensing Program Analyst (LPA) Lilia Hernandez conducted an unannounced inspection on 04/25/2025 at the facility for the purpose of inspecting the facility due to Licensee submitting an application for a capacity increase. LPA arrived at the facility at 2:20PM and met with Haykush Khachatryan, Licensee. LPA observed 8 children and 1 Assistant upon arrival. The purpose of the visit was to ensure that health, safety and personal rights as required by Title 22 and Health and Safety Regulations governing California Child Care Homes will be met. On 12/05/2024 LPA Roberto Luque Avila conducted an unannounced annual inspection. On 12/05/2024, Licensee was issued 6 deficiencies and was advised to submit Plan of Corrections (POCs) by 01/10/2025. POCs were cleared by LPA Roberto Luque Avila on 01/09/2025. During todays inspection, LPA did not observe a copy of the LIC9151 and LIC9149 Property Owner Notification and Consent on file. Licensee disclosed to LPA that they are the owner of the property. LPA advised to submit proof of control of property. LPA also observed Licensee's mandated reporter training to be incomplete. Licensee is missing the child care provider section of the training. The following deficiencies listed on the attached deficiency page are being cited in accordance with California Code of Regulations Title 22. Deficiencies that are being cited need to be cleared to protect the children\342\200\231s health & safety. ---Page 1 of 2 Prior an increase in capacity, the following is required, and the licensee will complete by 05/09/2025: Licensee will need to submit proof of control of property. Licensee will need to submit a copy of the mandated reporter certificate showing completion of both general and child care provider training. A large family child care license with a capacity of 14 may be granted upon review of application by licensing program manager. Once licensed, the licensee is required to adhere to the terms and limitations stated on the license. The Notice of Site Visit must remain posted for 30 days during the hours of operation after each site visit by a licensing representative. Failure to maintain posting as required will result in a civil penalty of $100.00. Exit interview conducted and report was reviewed with Haykush Khachatryan, Licensee. --Page 2 of 2
