What the inspector wrote
On 01/23/2024 Licensing Program Analysts (LPA) Laticia Thompson met with Applicant, Theonie Chandrasena and Dillini Weerasekara, Director. The purpose of the visit was an announced pre-licensing inspection at the facility to ensure that health, safety and personal rights as required by Title 22 and Health and Safety Regulations governing California Child Care Centers will be met. There were 22 children present during this inspection. This is an application for a Single License with a capacity of 15 infants and 30 Preschoolers. Infant ages are 2 months-24month and Preschoolers ages are 2 years into entry of 1
st
grade. The child care center will operate Monday – Friday 7:00 a.m. – 6:00 p.m. The facility is located on the grounds of Community of Christ church grounds. The fire clearance was approved on 07/14/2023. LPA toured and measured the on-limits areas. The facility has a total of 5 classrooms. Preschool children classrooms are room#1, room#4 and room#5 and will have access to 2 sinks and 2 toilets. Infants classrooms are room#2 and room #3 and will have access to 1 sink and 1 toilet.
Infant Indoor measurements are

The Indoor total measurements contain a total of 432.59 divided by 35 square feet, will not accommodate Applicant’s request for 15 children. Based on today’s measurement the facility capacity limit for indoor space is 12 infants.
Preschool Indoor measurements are:
The Indoor total measurements contain a total of 1,414.25 divided by 35 square feet will accommodate Applicant’s request for 30 Preschool children.

Infant Outdoor Activity measurements are:
The outdoor activity space total measurements contain a total of 1270.94 divided by 75 square feet, will accommodate Applicant’s request for 15 children.
Preschool Outdoor Activity measurements are:
The outdoor activity space total measurements contain a total of 1,729.29 divided by 75 square feet, will not accommodate Applicant’s request for 30 Preschool children. Based on today’s measurement the facility capacity limit for outdoor space is 23 Preschool children.
Indoor/Children’s Area:
The following was observed.
Parents will enter and exit through the gate located next to the parking lot area. There is a doorbell located on the gate to reach staff and gain access to the facility.
Parents will sign in and out electronically under the bright wheel
application. For best practice purposes, LPA advised facility there should be a plan in place on how the records would be backed up, and how they would be made available in the event of a power failure or disaster.
Children restrooms are located next to the Preschool outdoor activity space with a play apparatus. Sinks provide water warm to touch. Staff restrooms are located behind classroom #3 and in the additional preschool outdoor activity space.
The facility has filtered water dispensers where children can fill their personal water bottles for outdoor and indoor activities.
There is an operable carbon monoxide detector located in classroom #1. LPA advised director to place carbon monoxide detectors in rooms #4 and #5 that are located in a bungalow. Page 2 of 7
LPA observed First aid kit with the required materials including additional thermometer.
Furniture and equipment were observed to be maintained in good condition, free of sharp, loose or pointed parts.
Tables and chairs were observed to meet the needs of the children. There are cots for napping.
There are 3 cribs and 2 changing tables located in the infant area.
LPA advised director that Cribs shall not limit the ability of staff to see the infant. Have a padded surface no less than one-inch thick and be covered with washable vinyl or plastic. Have raised sides at least three inches high
.
All play equipment and materials used by children were observed to be age-appropriate. Director will provide age guidelines for apparatus.
There are cubbies in all classrooms for storage of children clothing and personal belongings.
There is a refrigerator located in the infant crib room that requires a thermostat to regulate the temperature.
The classrooms have central air for heating and cooling.
The classrooms were clean in good repair.
Windows were in good repair free of chipping paint, dirt, insects, or debris.
Classrooms have adequate lighting.

The facility is equipped with landline telephones service located in the office and classroom #1. There are surveillance cameras throughout the facility.
Medication is stored in the restroom located in classroom #3 (crib room). Director will replace storage cabinet with a cabinet that requires a key for access.
Disinfectants, cleaning solutions, and other items that are dangerous to children are stored in a cabinet located in kitchen. Director will place a safety latch on cabinet.
The program provides lunch and snacks.
The kitchen and food-preparation storage areas were observed to be clean and free of litter and rubbish. The refrigerator located in the kitchen has a thermostat to maintain temperature regulation. Menus are in writing and are posted in advance in an area accessible for review by the child's authorized representative. Copies of the menus as served shall be dated and kept on file for at least 30 days. Menus shall be made available for review by the child's authorized representative and the Department upon request.
The required postings were not posted for parents and visitors to review. Each licensee shall have a disaster and mass casualty plan of action.
Ill children will use the office area and staff restroom for isolation.
LPA observed 3 fire extinguishers size 2A10B:C with an inspection date of 1/19/2023.
LPA observed trash cans with tight fitting lids.
LPA did not observe any bodies of water on the premises.

Outdoor
The entire outdoor activity grounds are enclosed by a fence to protect children and to keep them in the outdoor activity area. The fence is at least four feet high. The surface of the outdoor activity space has artificial grass for cushioning to prevent injury from falls. The outdoor activity space was observed to be maintained in a safe condition and wasn't free of hazards. There is adequate shade in the outdoor activity area. LPA observed shade for resting.
The following items were discussed:
The director and applicant was reminded that all adults 18 and over responsible for administration or direct supervision of staff, persons who provides care and supervision to children, and staff who have contact with children, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.
LPA reviewed with facility representative the LIC 311A, Records To Be Maintained At The Facility, for child’s records, personnel records, administrative records, and documents to be posted.

Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018,
requires all licensed Child Care Centers (CCCs) constructed before January 1, 2010, to test their water (used for drinking and food preparation) for lead contamination before January 1, 2023, and then every 5-years after the date of the first test.
For child care center licenses issued after July 1, 2022, the licensee shall test their water for lead within 180 days of licensure pursuant to Written Directives section 101700 (PIN 21-21.1-CCP). Child Care Center testing deadline has not past.

The applicant and director was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California.
Community Care Licensing Division (CCLD) regularly sends information to licensed facilities, providers, and stakeholders by way of Provider Information Notices (PIN), Program Quarterly Update Newsletters and other important information communication platforms. LPA discussed the safe sleep regulations with applicant and direcetor and provided the Child Care Licensing Safe Sleep webpage at

as an additional resource. LPA also informed facility representatives of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at

and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.
The following corrections are needed before a licensure will be considered.
Provide a request for a waiver for preschool activity space.
Provide pictures of cabinet for storage of medication.
Place a safety latch on cabinet in Kitchen area.
Place a thermostat in refrigerator located in the crib room.
Provide pictures of required posting for parent and authorized representatives to review.
Provide proof of fire inspection for fire extinguishers.
Provide Play Apparatus age group.
Provide a copy of Mass disaster plan.
Remove storage cabinet in room #2
Place cushioning around TV edges.
Place safety knobs on doors.
Place a trash can in room #2
Remove debris on infant play yard area.
Corrections are due by 02/23/2024.
An exit interview was conducted with plan of corrected items discussed. LPA provided Dillini Weerasekara, (Director) with appeal rights and a copy of this report. Final license determination will be made upon review by the Licensing Program Manager and corrections.
