What the inspector wrote

Licensing Program Analyst (LPA) Tatiana Bickham conducted an unannounced Plan of Correction (POC) Visit on 10/15/2025 at 9:05 AM. Upon arrival, LPA disclosed the purpose of the inspection and met with Director Shannon Kelly, who guided the LPA on a tour of the facility. There were 4 infants and 1 preschool child present during today’s inspection. LPA also observed 2 staff present at the time of this inspection. During the Annual Required Inspection conducted on 9/17/25, the following deficiencies were issued; Type B - 101174(a ) / Disaster and Mass Casualty Plan. Type B - 101439(b) / Changing tables. Type B - 101238(a) / Facility shall be clean and in good repair. Technical Violation- 1596.954 HSC- Facility shall have a carbon monoxide detector. Technical Violation- 101239(n) CCR- Furniture and equipment shall be free of loose, sharp, or pointed parts. Technical Violation- 101429(a)(2)(C)3 CCR- 15-minute infant safe sleep documentation. During today's visit, LPA observed the Disaster and Mass Casualty Plan, a carbon monoxide detector, and 15-minute safe sleep documentation for infants available both on Brightwheel and in paper form. LPA did not observe any furniture or equipment with sharp, loose, or pointed parts. LPA observed a different mat on the changing table however, the mat was not free of tears. LPA informed the Director that all changing tables must have new, intact mats. The Director confirmed that new mats had been ordered, and LPA observed a receipt for the order with an estimated arrival date of 10/16/25. Page 1. The infant room appeared more organized compared to the previous visit. LPA is requesting additional changes to the feeding/classroom area within the infant room. LPA advised the Director to remove any items from the table and cubbies that are unnecessary or not being used by the children. The technical violations and deficiencies noted above have been cleared except for the deficiency 101238(a) / Facility shall be clean and in good repair. The Notice of Site Visit was provided and must remain posted for 30 days during the hours of operation after each site visit by a licensing representative. Failure to maintain posting as required will result in a civil penalty of $100.00. Exit interview was conducted with Shannon Kelly and Appeals Rights provided. Page 2.
