What the inspector wrote

On 09/18/2025 at 09:25 AM, Licensing Program Analyst (LPA) Elicia Calvillo conducted an unannounced Plan of Correction (POC) Visit. LPA identified self and the reason for the visit. LPA met with Lia Ovsepjanova, Licensee who guided LPA on a tour of the inside and outside of the facility. There was a total of 8 children present, 2 infants and 6 preschool ages. During today's visit, LPA cleared the Plan of Correction for the following deficiencies, initially cited during a visit on 09/10/2025. One Type A Deficiency for 102416.5(c)Staffing Ratio and Capacity and Two (2) Type B Deficiencies for sections 102370(d)(1) Criminal Record Clearance and 102421 (b) Child's Records. Also, LPA cleared the Plan of Correction for the following deficiencies, initially cited during a visit on 03/06/2025. One (1) Type A Deficiency for section 102416.5(c)Staffing Ratio and Capacity and five (5) Type B Deficiencies for sections 102425(j)(1) Infant Safe Sleep, 102417(g)(7) Operation of Family Child Care Home, 102419(d)(1) Admission Procedures and Authorized Representatives Rights, 102417(m)(3) Operation of Family Child Care Home, and 102425(c) Infant Safe Sleep. LPA provided Lia Ovsepjanova, Licensee with Letter of Deficiency Citations Cleared for the deficiencies listed above, all the citations have been cleared and no further action is required. LPA observed that the Licensee is maintaining their staffing ratio and capacity as specified on their Facility License. LPA conducted a review of the children\342\200\231s records. Page 1 of 2 Licensee request for a capacity increase is pending Department approval. A Notice of Site Visit was given and must remain posted for 30 days during the hours of operation after each site visit by a licensing representative. Failure to comply with posting requirement will result in an immediate civil penalty of $100.00. Exit interview was conducted with Lia Ovsepjanova, Licensee including, but not limited to Provider Rights, Appeal Procedures and Agencies Consultative Role.
