Follow-up on corrections·September 9, 2025

On 09/09/2025, Licensing Program Analyst (LPA) Amelia Morales conducted an unannounced Case Management visit to the facility above. Upon arrival, LPA Morales was greeted by Licensee Mariam Boyajyan, and was guided on a tour of the facility. Armenian interpreters services were provided. There were 3 children present at the time of the arrival. During the time of the inspection, LPA Morales observed the following; in reviewing the staff associations, it was noted that the following staff are not associated to the facility; Armine Hovhannisyan, does have fingerprint clearance. Per Licensee, Armine Hovhannisyan began working at the Family Child Care Home as of 9/02/2025.\302\240A Type B citation is being issued and a $500 Civil Penalty is being assessed. In reviewing staff association Sona Arakelyan does not have fingerprint clearance. Per Licensee Sona Arakelyan has been working at the facility for a month and a half. Per Licensee, Sona Arakelyan went to obtain fingerprint clearance. Licensee Boyajyan, provided documentation of the Request for Live Scan Services for Sona Arakelyan. However, upon LPA Morales inspection of the form, LPA Morales noted to Licensee that the form that was filled out was the incorrect form. LPA provided a copy of the correct form.\302\240 A Type A citations is being issued and a $500 Civil Penalty is being assessed. LPA Morales informed Licensee Mariam Boyajyan that this report dated (09/09/2025) documents (one type A CITATION) Type A citation(s) which shall be posted for 30 consecutive days as there is/are immediate risk(s) to the health, safety, or personal rights of children in care. Page 1 of 2 LPA Morales informed the Mariam Boyajyan to provide a copy of this licensing report dated (9/9/2025) that document any Type A citation(s) to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification.
