What the inspector wrote

On 10/05/2023, Licensing Program Analyst (LPA), Adrian Risher conducted a pre-licensing inspection for a relocation of a family child care home . LPA met with applicant Seline Alaverdyan and toured the facility. The applicant is the only individual present at the time of the inspection. LPA did not observe any children present. The home is clean, orderly and comfortable. All adults in the home have been fingerprinted. Applicant is waiting for her clearance to be transferred and associated to this facility number. The hours of operation will be 7am to 5:30pm Monday thru Friday. Applicant is requesting to have a family child care home with a capacity of 14. The fire clearance was approved on 08/20/2023 by Inspector Karina Wakefield. The single family home has 1 bedroom and 1 bathroom. The home is located in the back of 1056 E Orange Grove Ave Burbank CA 91501. Applicant stated the families will enter the front entrance of the home which is located in the alleyway. The daycare will consist of the living room, bedroom and bathroom. These areas were inspected by LPA. The other rooms are considered off-limits and inaccessible. The living room will be used for daily activities and eating. The bedroom will be used for napping. Mats will be used for napping. The bedroom will be used as the isolation area. Applicant is renting the home. Applicant provided property owner/landlord consent form with the application. LPA inspected the bathroom and did not observe any medications or poisons that could pose a potential risk to children in care. The kitchen was inspected during the visit. LPA did not observe any knives or sharp objects, detergents or cleaning supplies that would pose a potential risk to children in care. These items were made inaccessible to the children. Applicant has placed a safety gate at the entrance of the kitchen. The outside play area is located in the back of the home. LPA did not observe any bodies of water. Applicant stated the outside play area will be off-limits until the next door neighbor completes their construction. Applicant will contact the fire department once she installs a fire-resistant tent in the backyard. LPA observed a charged fire extinguisher (2-A:10-B:C). LPA tested the smoke detector and carbon monoxide detector. Applicant has a first aid kit which includes band aids and a thermometer. The home has central heating and air. Applicant has current CPR, first aid, health and safety which expires 04/2025. Applicant has provided immunizations and mandated reporter certifications. Per the applicant, there are no firearms on the premises. Applicant reports that there are no pets in the home. LPA did not observe any baby walkers, exersaucers or bouncers. The parent board will be posted for parents to view required information such as the license, parent's rights poster, personal rights, and emergency disaster plan. A copy of the children's roster will be kept accessible. The following corrections were discussed with applicant in order for the home to be ready for conducting child care: 1. Transfer Criminal Record Clearance to new license A license to operate a Large Family child care home will be issued following final administrative review and receipt of outstanding corrections needed. No license will be issued today 10/05/2023. Applicant will notify LPA when corrections have been made and the Applicant has 30 days to complete corrections. Applicant was reminded that all adults 18 and over living in the home, persons who provide care and supervision to children, and staff who have contact with children, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day for a maximum of 5-days or, if the penalty is for a repeat violation, for a maximum of 30-days per person will be assessed if this regulation is violated. The applicant provided proof of control of property. Because the applicant rents/leases the home, proof of landlord notification is required. The LPA observed the Property Owner/Landlord Notification form (LIC9151) that the applicant confirms was provided to the property owner/landlord. The applicant obtained a signed Property Owner/Landlord Consent form (LIC 9149). Incidental Medical Services (IMS) policy was discussed. For IMS information see PIN 22-02-CCP . When any IMS is provided, a Plan for Providing IMS must be submitted to the Department. LPA reviewed with applicant the LIC 311D, Forms/Records to Keep In Your Family Child Care Homes, children\342\200\231s forms/records, facility forms/records, and information to be posted. Entrance Checklist was provided to the applicant. PIN 22-05-CCP Page Eight LPA discussed the safe sleep regulations with applicant and discussed the Child Care Licensing Safe Sleep webpage at: https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep , as an additional resource. LPA also informed applicant of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at: https://www.cpsc.gov/ , and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment. On this date, 08/09/2023, the California Attorney General - Megan\342\200\231s Law website was searched for information on sex offenders required to register with local law enforcement under California's Megan's Law. No registered sex offenders were found at the facility addresses. Under state law, some registered sex offenders are not subject to public disclosure; therefore, they may not have been included in this search. However, the Department conducts a monthly cross reference of each address on record for all registered sex offenders against all CCLD facility addresses pursuant to information shared by California DOJ. Applicant was informed of the MyChildCarePlan.org site, a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California. Community Care Licensing Division (CCLD) regularly sends information to licensed facilities, providers, and stakeholders by way of Provider Information Notices (PIN), Program Quarterly Update Newsletters and other important information communication platforms.
