What the inspector wrote

At 8:05AM Licensing Program Analyst (LPA) Angela Luz and Licensing Program Manager (LPM) Emiko Bell met with Licensee Goar Karapetian to conduct an unannounced annual random inspection for the purpose of a change in capacity. Also present for this inspection was 3 preschool aged children. The home was toured to conduct a Health and Safety Inspection. Entrance checklist for Family Child Care Home (FCCH) was provided. Required documents are posted in a prominent, publicly accessible area of the facility. Days and hours of operation are Monday-Saturday 6:30AM - 9:30PM. Licensee's primary languages are Armenian/Russian. The visit was conducted in English. The OFF LIMIT AREAS are all bedrooms, bathroom in hallway, and kitchen. The ON LIMIT AREAS are the accessory room, dining room, living room, and bathroom adjacent to living room. OFF LIMIT areas are inaccessible by doors with child locks on them and visual supervision. The home is neat and clean with heating and ventilation for safety and comfort. The ISOLATION area is the living room. The outdoor play area is free from defects or dangerous conditions and is fully fenced. All hazardous materials and toxins are kept out of reach of children. Licensee states that any poisons are stored in the off limit bathroom. Licensee states there are no firearms in the home. The home has a fully charged 2A10BC fire extinguishers, working smoke detector, working carbon monoxide detector, and working telephone. LPA reminded Licensee that fire extinguishers should be purchased or serviced once a year. There is a fireplace in the living room that is barricaded. LPA observed security cameras around the facility that records video and audio. There are ample age-appropriate toys that are in safe and good condition. Currently, all children in care sleep on cots. Infants in care are able to climb out of a play yard or crib. Licensee has play yards that are currently put away. LPA reminded there shall be one crib or play yard for each infant who is unable to climb out of the crib or play yard. Licensee uses talking with the child as a form of discipline. Licensee understands that children's personal rights should not violated, including but not limited to, no corporal punishment, interference with eating, intimidation, or other actions of a punitive nature. Children are treated with dignity, receive safe, healthful, and comfortable accommodations. Licensee Pediatric CPR and First Aid certificate is current and expires on 9/3/25. Licensee Mandated Reporter Training for Child Care Providers is current and expires on 9/9/25. Licensee was reminded that Pediatric CPR/First Aid and Mandated Reporter certifications need to be renewed every two years. A copy of licensee's immunization is on file. Licensee stated they conduct fire and disaster drills. LPA reminded that fire and disaster drills need to be practiced and documented at least every 6 months. Licensee does not have liability insurance for the FCCH and A ffidavit Regarding Liability Insurance For Family Child Care Home (LIC 282) was not found in children's files. LPA reminded that Licensee can choose to obtain liability insurance for the FCCH or have authorized representatives sign LIC 282. LPA reviewed 5 children files and 2 staff files for required documents. All files reviewed today have the required documents. LPA reminded that children's immunization records should be kept up-to-date, and transcribed on the California School Immunization Record (PM 286). Documentation of infant sleep checks every 15 minutes that notate any signs of distress are maintained. Licensee was reminded that all adults 18 and over living or working in the home, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated. LPA discussed the safe sleep regulations with licensee and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and\302\255resources/safe-sleep as an additional resource. LPA also informed licensee of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment. Incidental Medical Services (IMS) policy was discussed. For IMS information see PIN 22-02- CCP. When any IMS is provided, a Plan for Providing IMS must be submitted to the Department. Licensee was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California. During the exit interview, the Licensee Goar Karapetian , confirmed that there are no Registered Sex Offenders living in the facility and LPA completed the RSO profile in FAS on 10/24/23. Two Type B deficiencies were issued today on attached page 809-D. Two advisory notes were issued. A notice of site visit was given and must remain posted for 30 days. Appeal rights provided. Exit interview conducted and report was reviewed with the Licensee Goar Karapetian .
