What the inspector wrote
On 05/31/24, beginning at 08:00 AM, Licensing Program Analysts (LPAs) Elicia Calvillo and Roberto Luque Avila conducted an announced Pre-Licensing inspection at 6920 White Oak Avenue, Lake Balboa, CA 91406. Applicant is applying for a Large Family Child Care Home, anticipating caring for 14 children from ages 2 months to 13 years of age. The applicant\342\200\231s current proposed hours of operation are Monday through Friday 7:30AM-4:30PM and Saturday 7:30 AM to 3:00 PM. LPAs met with Vanessa Nunez, Applicant, who guided LPAs on a tour of the inside and outside of the home. There were no other adults present.
The applicant provided proof of control of property.
As drawn on the facility sketch and observed during the tour, the facility is a one story residence consisting of a living room, dining room, kitchen, master bedroom, 3 bedrooms, 3 bathrooms, laundry closet and detached garage. The outdoor play area is fenced and there is a fenced pool.
LPAs observed the following inside areas will be used for child care: living room, dining room, master bedroom and 1 bathroom will be used for child care activities. Applicant stated they will set up the toys and play items, LPAs observed toys and play items stored in boxes are safe, clean, and appropriate for the ages of the children.
Bathroom#1 located in hallway near bedrooms will be used for children. The vanity cabinet holds paper products and no hazards were observed. Electrical outlets have safety covers installed.

Master bedroom will be used for infant sleeping area and isolation when a child is ill; room is equipped with 1 crib and changing table applicant stated they will add an additional crib/play yard. Applicant stated that two additional mats will be purchased.
Each bedroom is equipped with a smoke detector and there is one combination smoke/carbon monoxide detector in the hallway; all were tested and are operable.
LPAs observed the following inside areas are not used for child care; kitchen, 3 bedrooms, 2 bathrooms, and laundry closet. Applicant stated they will install a gate for the laundry closet and will change the bedroom door knobs so they will lock.
Per applicant, there is firearm and ammunition on the premises and LPAs observed the firearm and ammunition are stored in separate secured areas. Applicant stated they will keep all poisons in a locked storage area and install a cabinet for storage in the detached garage. No poisons were observed in the home during the inspection. LPAs observed detergents, cleaning compounds, medication and other hazardous items and applicant stated they will make items inaccessible with the use of safety latches.
There are two pets on the premises. Applicant stated the pets will be housed in the side yard during the hours of operations and they will install a gate (photo taken).
Fire Clearance was completed on 05/02/2024. There is a working fire extinguisher, combination smoke detector/ carbon monoxide detector and adequate heating and ventilation for safety and comfort. LPAs did not observe a fireplace or an open face heater.
LPAs observed the following outside areas: the backyard can be accessed from living and dining room french doors through Kitchen area, which will remain locked. Backyard can also accessed through the French door in the master bedroom, which will remain locked. Play area accessible to children is fenced and applicant will install another fence to section the play area off from the rest of the backyard. Applicant stated they will purchase additional outdoor toys and equipment and LPAs observed a grass and cement area with a few play items that are safe and appropriate for the ages of the children.

LPAs observed a pool. The fence is designed and installed so children could not remove any portion of it, it completely surrounds the pool, is constructed so that it does not obscure the pool from view, no window or door provides direct access to the pool. Fence is at least 5 feet high (photo taken). The opening between the railing does not exceed 4 inches. The gate swings away from the pool, and the gate does not self close or self-latch. Applicant stated they will repair the pool gate in order for it to self close and self latch (photo taken).
Vanessa Nunez, Applicant, agrees on the LIC 809 report that the fencing will remain in place whenever licensed care is provided, and so long as the mesh fence makes the swimming pool inaccessible to children as determined by licensing staff.
Front yard is used as a walk way when children enter the home.
LPA reviewed with applicant the LIC 311D, Forms/Records to Keep In Your Family Child Care Homes, children\342\200\231s forms/records, facility forms/records, and information to be posted. Entrance Checklist was provided to the applicant.
Community Care Licensing Division (CCLD) regularly sends information to licensed facilities, providers, and stakeholders by way of Provider Information Notices (PIN), Program Quarterly Update Newsletters and other important information communication platforms. Reviewed reporting and notification requirements for any unusual incident or injury to the Child Care Regional Office by telephone within 24 hours and within 7 days complete a written report, LIC 624B Unusual Incident/Injury Report
Reviewed only children eating may be in highchairs and that car seats are utilized only for transportation.
Reviewed regulation prohibits smoking in a private residence licensed as a family child care home.

Reviewed fire extinguisher type 2A:10BC must be serviced annually. Smoke and carbon monoxide detectors should be checked, and batteries should be replaced.
Criminal Record Clearance - Family Child Care Homes Vanessa Nunez, Applicant was reminded that all adults 18 and over living in the home, persons who provide care and supervision to children, and staff who have contact with children, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day for a maximum of 5-days or, if the penalty is for a repeat violation, for a maximum of 30-days per person will be assessed if this regulation is violated.
LPAs discussed the safe sleep regulations with applicant and discussed the Child Care Licensing Safe Sleep webpage at https://

\302\255 resources/safe-sleep as an additional resource. LPA also informed applicant of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at

and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.
LPAs did not observe the following items during the inspection: Infant Walkers, Johnny Jumpers, Saucer Chairs, Trampolines and/or any other item that fall into these categories are not permitted in a family child care facility.
Applicant was reminded that children in care are to be supervised at all times and made aware that children shall not be left in parked vehicles. Car seats are used for transportation purposes only and are not used for sleeping children.

Applicant was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California.

the California Attorney General - Megan\342\200\231s Law website was searched for information on sex offenders required to register with local law enforcement under California's Megan's Law. No registered sex offenders were found at the facility addresses. Under state law, some registered sex offenders are not subject to public disclosure; therefore, they may not have been included in this search. However, the Department conducts a monthly cross reference of each address on record for all registered sex offenders against all CCLD facility addresses pursuant to information shared by California DOJ.

The following list of corrections needs to be completed prior to licensure approval:
Pending Criminal Record Clearance needs to be completed for Adult living in the home.

Secure kitchen cabinets with child proof latches and safety gate.
Add additional crib/play yard for infant.
Add 2 additional sleeping mats.
Add additional outdoor play equipment.
Secure outdoor heaters and bbq grill on side of detached garage with a gate.
Add side yard gate for pet area.
Add fence to separate the play yard from rest of the back yard.
Applicant to correct the above issues by 06/17/2024.
