What the inspector wrote

On 09/04/2025 at 03:45 PM, Licensing Program Analyst (LPA) Elicia Calvillo conducted an unannounced Case Management -Health & Safety Check visit. LPA identified self and met with Valeria Quiros, Assistant, who guided LPA on a tour of the inside and outside of the facility. LPA observed 1 infant, 7 preschool, 2 school age children in care and 2 assistants at the facility.. Upon arrival Ayaz Nabiyev, Licensee was not present at the facility. Per staff, he was not present at the facility since the facility opened at 06:57AM. At approximately, 03:50PM, Valeria Quiros, Assistant received a telephone call from Licensee Ayaz Nabiyev and gave the telephone to LPA to speak with Licensee. Per Licensee, they just left the facility to take their grandmother to an appointment. Licensee stated, "Do you want me to return?", LPA advised applicant to return if they were available or they could have Valeria Quiros, Assistant complete the visit. LPA observed that the health and safety of children were being maintained by the facility staff without the Licensee present. A Notice of Site Visit was given and must remain posted for 30 days during the hours of operation after each site visit by a licensing representative. Failure to comply with posting requirement will result in an immediate civil penalty of $100.00. Exit interview was conducted with Valeria Quiros, Assistant including, but not limited to Provider Rights, Appeal Procedures and Agencies Consultative Role.
