Follow-up on corrections·October 7, 2025

On 10/07/25, @ 8:15am Licensing Program Analyst (LPA) Jeanine Lipsey conducted an unannounced Case Management - Deficiencies visit. Census 5 children in care being supervised by 2 adults. Staff disclosed licensee was not present when they arrived at 7:15am. LPA called licensee at 9am and left a voice message stating LPA was at the childcare waiting on them. At 9:06 am LPA text licensee with the same message. At 9:11am Licensee called back stating, they just dropped off a relative and will arrive in 20 minutes. Licensee arrived at the home at 9:35am. Licensee was not present when LPA's arrived on previous dates of 5/15/25, 7/20/25, 9/4/25 and 9/19/25. LPA observed the home to have a sign outside of the home with License number 197493132. Licensee explained the sign was from previous day care that they owned. Licensee stated they were aware of the incorrect number on the sign and had plans to correct it by next week. Licensee disclosed they were told previously regarding the sign but was too busy to fix it. LPA observed the sign to still be up on 10/7/25. Based on LPA's observations, civil penalties listed on (LIC421FC) and the following deficiencies listed on the attached LIC809D (deficiency page) are being cited in accordance with Title 22, Division 12, Chapter 3, of the California Code of Regulations. Deficiencies that are being cited need to be cleared to protect the children's health & safety. Exit interview conducted and report was reviewed with Licensee Ayaz Namiyev. A notice of site visit was given and must remain posted for 30 days.
