What the inspector wrote

On 1/26/26 at 11:33am AM, Licensing Program Analyst (LPAs) Jeanine Lipsey and Christina Nunez conducted an unannounced case management visit for the purpose of and delivering an amended report dated 12/2/25 to separate the deficiencies and obtain signatures. LPAs met with, Staff Valeria Quiros and toured the facility. Upon entry, LPA Lipsey observed 6 children, of whom 1 was was an infant, being supervised by 2 staff members. At 1 pm one staff member left for lunch break. At 2:36pm staff returned from break. During visit Licensee was not present. Per staff the licensee was not present when the staff arrived at 6:50am and had not been present today. Based on interviews conducted, Staff #3 make children go into the bedroom alone as a form of punishment. Type B violations will cited and civil penalties assessed for repeat violation. Exit interview conducted and report was reviewed with Staff Valeria Quiros. A Notice of Site Visit was given and must remain posted for 30 days.

This requirement is not met as evidence by: Based on observation, Licensee was not home when LPA arrived at 11:33am. Licensee was not present at 6:50am when staff arrived, which poses/posed a potential health, safety or personal risk to children in care.
