What the inspector wrote

On 9/29/25 at 9AM, Licensing Program Analyst (LPA) Angela Luz met with Staff Margarit Janpoladyan (S1) to conduct an annual random inspection. Licensee submitted documentation for an increase in capacity to a large Family Child Care Home. S1 informed LPA that Licensee Hovhannes Hovhannisyan is expected to return to the facility in about one hour. S1 toured LPA around the facility and provided LPA with records to review. Licensee arrived around 9:50AM. Also present for this inspection is six preschool aged children and one other staff. All adults present are fingerprint cleared and associated to the facility. Days and hours of operation are Monday to Friday 7AM - 5PM. Ages to be served are 1 year old - 12 years old. English and Armenian are spoken to children in case. Physical Facility: S1 toured the on limit areas of the facility with LPA. The facility shares the backyard with 2510 N. Myers St (main house). The Department has a written Declaration on file that Licensee lives at the facility address (2512 N. Myers Str.) Entrance to the facility is by the side gate to the right side of the property. The ON LIMIT AREAS are the living room, bedroom, bathroom, and backyard. The OFF LIMIT AREAS is the kitchen, main house, patio, and outdoor kitchen. Licensee uses visual supervision to keep children in care out of the kitchen. LPA observed child locks on all lower kitchen cabinets and drawers to prevent access from children in care. The patio has closed, lockable curtains with furniture behind them and the outdoor kitchen has closed, lockable curtains with a child gate to prevent access from children in care. The home is neat and free of debris with heating and ventilation for safety and comfort. The ISOLATION AREA is the bedroom. The outdoor play area is free from defects or dangerous conditions, fully fenced, and has shade available. There are no bodies of water. All hazardous materials and toxins are kept out of the reach of children. Licensee understands that any materials that are labeled \342\200\234Keep out of reach of children\342\200\235 or have similar messaging should be kept out of reach from children in care. Licensee states that there are no firearms in the home. The home has a fully charged 2A10BC fire extinguisher that was purchased in 12/2023, working smoke detector, working carbon monoxide detector, and working telephone. LPA reminded that fire extinguishers should be serviced or newly bought once a year. Care and Supervision : There are ample age-appropriate toys that appear to be safe and in good condition. Licensee states they use talking with the child and time out as forms of discipline. LPA reminded that best practice when giving time outs is no longer than one minute per year of age. Licensee understands that children's personal rights should not be violated, including but not limited to, no corporal punishment, interference with eating, intimidation, or other actions of a punitive nature. Licensee understands that children are treated with dignity, receive safe, healthful, and comfortable accommodations. LPA reminded that Licensee is required to be present for 80% of operating hours per day. Licensee understands how to report Unusual Incidents/Injuries. LPA reviewed and gave copies of Title 22 Section 102425 Infant Safe Sleep with Licensee. Licensee understands Infant Safe Sleep requirements. LPA reviewed Staffing Ratio and Capacity regulations with Licensee. Licensee understands requirements to run a Large Family Child Care Home. Record Review: Licensee Pediatric CPR/First Aid expires 7/2026 , and Mandated Reporter Training for Child Care Providers expires 7/26/27. Licensee was reminded that Mandated Reporter Training (AB1207) and CPR/First aid certifications needs to be renewed every two years. A copy of the licensee\342\200\231s immunization is on file. The Licensee conducts and documents fire and disaster drills every six months, last drill was on 9/22/25. A current copy of the facility roster was viewed, and a copy was obtained. Licensee does not carry liability insurance. Signed copies of Affidavit Regarding Liability Insurance (LIC 282) were found in children\342\200\231s files. Entrance checklist for Family Child Care Home was provided and licensee was reminded of documents to be posted in a prominent, publicly accessible area of the facility. LPA reviewed six children files and three staff files during today's inspection. LPA reminded that children's immunizations shall be kept up to date. Staff files were complete. Licensee was reminded that all adults 18 and over living or working in the home, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated. LPA discussed the safe sleep regulations with licensee and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-andresources/safe-sleep as an additional resource. LPA also informed licensee of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment. Incidental Medical Services (IMS) policy was discussed. For IMS information see PIN 22-02- CCP. When any IMS is provided, a Plan for Providing IMS must be submitted to the Department. Licensee was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California. During the exit interview, the LICENSEE, Hovhannes Hovhannisyan , confirmed that there are no Registered Sex Offenders living in the facility and LPA completed the RSO profile in FAS. Megan's Law was checked on 9/25/25. LPA informed Licensee a Large Family Child Care Home will be approved pending Licensing Program Manager review. No deficiencies are issued today. 2 advisory notes are issued. Appeal rights were provided. A notice of site visit was given and must remain posted for 30 days. Exit interview conducted and report was reviewed with the licensee Hovhannes Hovhannisyan .
