What the inspector wrote

On 10/8/24, at 9:15am Licensing Program Analyst (LPA) Jeanine Lipsey conducted an unannounced Case Management incident Visit to this facility. Upon arrival, LPA met with Director Melanie Martin, who guided LPA on a tour of the facility. There were twenty nine children being supervised by seven staff. LPA explained the purpose of today's visit is to follow-up on an incident that was reported to Community Care Licensing (CCL) on 9/27/24. The incident that occurred on 9/25/24, was reported to the Department on 9/27/24, via telephone. The facility reported the Unusual Incident to the Department within the required 24 hours of occurrence. Information reported to the Department indicated that Child #1's personal rights may or may not have been violated. During today's visit, LPA conducted interviews, took photos of the area in question, and also obtained the parents information. LPA observed the slide in the play area. The slide was age appropriate and deemed safe for the children to play on. The following steps will be in place to ensure the safety of the children: 1. Staff will make sure toys will not be at the bottom of the slide. 2. Staff will stand by the slide and remove the toys, if the kids put them there. 3. Staff will talk to the children to remind them about not putting the toys near the slide. 4. Director will schedule a safety seminar with all the staff. The agenda will include taking the staff around the grounds and show them pointers of potential hazards to stop a problem before it starts. Based upon information received and the interviews conducted, it was determined that the personal rights of Child #1 were not violated. No additional follow up is necessary regarding the incident reported. There were no deficiencies cited during today\342\200\231s inspection. Director Melanie Martin was provided with The Notice of Site Visit (LIC 9213) \342\200\223 which must remain posted for 30 days during the hours of operation after each site visit by a licensing representative. Failure to maintain posting as required will result in a civil penalty of $100.00. An exit interview was conducted with Director Melanie Martin and Appeals Rights provided.
