What the inspector wrote
On 04/29/2024 Licensing Program Analysts (LPA) Laticia Thompson met with Mujtaba Noorzay, Uranus Noorzay (Applicants) and Vanessa Dione, (Director).The purpose of the visit was an announced pre-licensing inspection at the facility to ensure that health, safety and personal rights as required by Title 22 and Health and Safety Regulations governing California Child Care Centers will be met. There were no children present during this inspection. This is an application for a Single License with a Total capacity of 98; 18 infants ages 0 – 24 months, 80 Preschoolers ages 2 years into entry of 1
st
grade. The child care center will operate Monday – Friday 7:00 a.m. – 7:00 p.m. LPA toured and measured the on-limits areas. The fire clearance was approved on 02/12/2024. The facility has a total of 8 classrooms. Classrooms 1 and 8 will be dedicated to the infant program, classrooms 2-7 will be dedicated to the preschool program. Infants will have access to 1 toilet and 3 sinks. Preschool children will have access to 4 sinks and 4 toilets.
The following was observed:
Parents will enter and exit through the main office located next to the front parking lot of the facility. Anyone entering through the gate must use their assigned magnetic key chain.
Parents will sign in and out electronically under the Brightwheel application. Providers are to ensure that such records are immediately printable upon request for inspection and audit by program staff. For best practice purposes, LPA advised facility there should be a plan in place on how the records would be backed up, and how they would be made available in the event of a power failure or disaster. LPA provided applicant with a copy of PIN 18-03.1-CCP
All rooms have camera monitoring devices. The facility will provide filtered bottled water through dispensers where children can fill their personal water bottles for outdoor and indoor activities.
There are operable carbon monoxide detector located in classrooms 1-8 and the main office. LPA observed fire extinguishers with an inspection date of 05/19/2023. LPA observed multiple First Aid Kits with required materials. There is a handheld digital thermometer.
Furniture and equipment were observed to be maintained in good condition free of sharp, loose or pointed parts.
Tables and chairs were observed to meet the needs of the children. There is a sufficient number of cots and mats for napping. LPA advised applicant that napping cots should be marked so that the sleeping side can be distinguished from the floor side.
All play equipment and materials used by children in the classrooms were observed to be age-appropriate.
There are cubbies in all classrooms for storage of children clothing and personal belongings.
The classrooms have individual air-conditioning and heating units for heating and cooling. The classrooms were clean in good repair. Classrooms have adequate lighting. Windows were in good repair free of chipping paint, dirt, insects, or debris. LPA observed trash cans with tight fitting lids. The facility is equipped with landline telephone service and walkie/talkies in the classrooms and office area.
Disinfectants, cleaning solutions, and other items that are dangerous to children are stored in a cabinet located in the staff restroom and staff lounge in room #8. The cabinets are inaccessible to children. Cabinets have safety latches .
Ill children will use the office and office restroom for isolation
.
The isolation area is equipped with a couch for ill children. LPA advised applicant that isolation area for infants shall be equipped with a crib, cot, mat or playpen for ill infants. The isolation area shall be supervised by staff at all times.
The program provides snacks. The kitchen and food-preparation storage areas were observed to be clean and free of litter and rubbish. The refrigerator located in the kitchen has a thermostat to maintain temperature regulation. LPA advised applicants that menus should be in writing and posted in advance in an area accessible for review by the child's authorized representative. Copies of the menus as served shall be dated and kept on file for at least 30 days. Menus shall be made available for review by the child's authorized representative and the Department upon request.
All foods or beverages that require storage at a temperature of 45 degrees will be stored in the refrigerator located in the kitchen. LPA observe a thermostat to regulate storage temperature.
Infant Rooms
Infant room #8
is not connected to the main classrooms of the facility. Inside room #8 there is a staff lounge located in a separate area of the infant room. There is a refrigerator in this area where infants refrigerated food items will be stored. There are 8 cribs and 1 changing table. LPA advised director that cribs shall not limit the ability of staff to see the infant. Have a padded surface no less than one-inch thick and be covered with washable vinyl or plastic. Have raised sides at least three inches high.
Infant room #1
is located next to the main office. This room contains activity space and a restroom. Inside the restroom there is a changing table next to the sink.
LPA reviewed and provided applicant with copies and postings of Title 22 regulations related to infant safe sleep, infant needs and service plans, staff ratios and infant sleeping equipment.
The required postings are posted in the main office and every classroom for parents and visitors to review. Facility does not have a disaster and mass casualty plan of action. The applicant was advised to develop and implement a written plan to record the administration of prescription and nonprescription medications, to inform the child's authorized representative daily when such medications have been given. Medication will be stored in the main office inaccessible to children. Medication that requires refrigeration will be stored in the refrigerator located in the kitchen inaccessible to children.
LPA did not observe any bodies of water on the premises. Applicant stated there are no firearms on the premises. The facility does not provide transportation.
Outdoor
The entire outdoor activity grounds are enclosed by a fence to protect children and to keep them in the outdoor activity area. The fence is at least four feet high. Some surfaces of the outdoor activity space has natural and artificial grass for cushioning to prevent injury from falls. The outdoor activity space was observed to be maintained in good condition There is adequate shade in the outdoor activity areas. LPA observed benches for resting.
The following items were discussed:
The director and applicants were reminded that all adults 18 and over responsible for administration or direct supervision of staff, persons who provides care and supervision to children, and staff who have contact with children, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.
LPA reviewed with applicant and director the LIC 311A, Records To Be Maintained At The Facility, for child’s records, personnel records, administrative records, and documents to be posted.

Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018,
requires all licensed Child Care Centers (CCCs) constructed before January 1, 2010, to test their water (used for drinking and food preparation) for lead contamination before January 1, 2023, and then every 5-years after the date of the first test. For child care center licenses issued after July 1, 2022, the licensee shall test their water for lead within 180 days of licensure pursuant to Written Directives section 101700 (PIN 21-21.1-CCP).
LPA verified that the lead testing was completed in accordance to the Written Directives outlined in

The applicants and director were informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California.
LPA discussed the safe sleep regulations with Director and applicant and discussed the Child Care Licensing Safe Sleep webpage at:

, as an additional resource. LPA also informed applicants and director of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at:

, and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.

Waiver Request, Notice of Incomplete Application Documents remove and replace mattress in one of the cribs per our discussion.
An exit interview was conducted with plan of corrected items discussed. LPA provided applicant with appeal rights and a copy of this report. Final license determination will be made upon review by the Licensing Program Manager and corrections.
