What the inspector wrote
On 07/15/2024, beginning at 01:15 PM, Licensing Program Analysts (LPAs) Elicia Calvillo conducted an announced Pre-Licensing inspection at 16055 San Fernando Mission Blvd, Granada Hills, CA 91344. Applicants is applying for a Small Family Child Care Home, anticipating caring for 8 children from ages 2 months to 12 years of age. The applicant\342\200\231s current proposed hours of operation are Monday through Friday from 08:00AM-10:00PM. LPA met with Tigran Khachatryan and Lilia Adunts, Applicants, who guided LPAs on a tour of the inside and outside of the home. There was 1 child and 1 other adult present.
Because the applicants rents/leases the home, proof of landlord notification is required. The LPA observed the Property Owner/Landlord Notification form (LIC 9151) that the applicants confirms was provided to the property owner/landlord. The applicants obtained a signed Property Owner/Landlord Consent form (LIC 9149).
Individuals who reside in the home were noted and discussed. Applicants was reminded that all adults 18 and over living in the home, persons who provide care and supervision to children, and staff who have contact with children, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day for a maximum of 5-days or, if the penalty is for a repeat violation, for a maximum of 30-days per person will be assessed if this regulation is violated.

As drawn on the facility sketch and observed during the tour. This is a property with a main house and an ADU on the same property. The applicants live in the main house of a one-story dwelling, first floor includes studio, small kitchen, combined living and dining room, kitchen, 2 bathrooms, 3 bedrooms, office, laundry area (side of house), and detached garage. The outdoor area includes a covered patio with a safety fenced play area.
The children use the bathroom in the hallway near the office. The vanity cabinet contains paper products. Applicants will provide food for children in care.
Per Licensee, areas off limits to children and parents include: kitchen, 1 bathroom, bedroom #1 and bedroom #3, and detached garage.
LPA observed the following inside areas will be used for child care: The dining / living room will be used for child care activities, eating, and sleeping. There are 6 chairs and 1 table cots, a storage closet for children\342\200\231s items. Toys and play items are safe, clean, and appropriate for the ages of the children. Bedroom #2 will be used for child care activities and sleeping and there are 2 tables and 8 chairs. Toys and play items are safe, clean, and appropriate for the ages for the children. Electrical outlets have safety covers installed. There is a fire alarm and carbon monoxide detector in the area and is in working condition and test was conducted.
The front yard will be used for children to enter and exit the facility and as an extra play area. The children will use the covered patio for outdoor play time. The outdoor play areas were both observed to be fenced. LPA observed that the front and back outdoor play yard has toys and other materials for children to play. LPA did not observe any objects that can pose a danger to children on the front or back outdoor yard. The licensee states that supervision is always provided. LPA observed and ADU with emergency access from a back yard gate. The ADU is a separate unit with the entrance facing the alley. Per Applicants, the ADU tenants only have access to the back yard during an emergency. LPA observed a locked fence gate (emergency exit).
All areas identified on the facility sketch that are accessible for children to use were inspected for safety, comfort, and cleanliness. There is telephone service via a landline. There is central ventilation and heating. Safe toys play equipment and materials were observed.

Detergents, cleaning compounds, medications, and other items which could pose a danger to children were observed to be inaccessible to children. Applicants states that there are no poisons in the home and understands that storage areas for poisons must be locked with a key or combination lock. The restroom that children will use was observed to be safe and sanitary.
The valve on the required 2A 10BC fire extinguisher indicates fully charged and was serviced on 04/23/2024.
LPA did not observe any pools, spas, hot tubs, fish ponds, or similar bodies of water during the inspection. Applicants states that there are no firearms stored in the home. LPA did not observe a fireplace or an open-faced heater.
LPA discussed the safe sleep regulations with applicants and discussed the Child Care Licensing Safe Sleep webpage at https://

\302\255 resources/safe-sleep as an additional resource. LPA also informed applicants of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at

and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.
The licensee and other personnel have completed training on preventive health practices including Pediatric First Aid and CPR. The applicants Pediatric First Aid and CPR both expire on 05/2026.
There are first aid supplies available. Applicants states that if a child shows signs of illness they shall be separated in the office room away from other children.
The applicants have proof of immunization against influenza, pertussis, and measles. LPA observed that the applicants do have proof of the Mandated Reporter AB 1207 compliant Child Care Training Certificate on file.
All homes shall conduct fire and disaster drills at least once every six months and document the date and time of each drill.

LPA did/did not observe the following items during the inspection: Infant Walkers, Johnny Jumpers, Saucer Chairs, Trampolines and/or any other item that fall into these categories are not permitted in a family child care facility.
LPA advised applicants that no children will be left in parked vehicles, that car seats shall only be used for transportation.
Applicants states there are a pet on the premises. During hours of operations the pet will be in bedroom #3 (off limits area) or outside the house.
Smoking is prohibited in a licensed Family Child Care Home. Per Applicants, no one smokes in the home.
Incidental Medical Services (IMS) policy was discussed. For IMS information see PIN 22-02- CCP. When any IMS is provided, a Plan for Providing IMS must be submitted to the Department. Applicants were informed of the MyChildCarePlan.org site, a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California.
On this date, 05/28/2024, the California Attorney General - Megan's Law website was searched for information on sex offenders required to register with local law enforcement under California's Megan's Law. No registered sex offenders were found at the facility addresses.
Under state law, some registered sex offenders are not subject to public disclosure; therefore, they may not have been included in this search. However, the Department conducts a monthly cross reference of each address on record for all registered sex offenders against all CCLD facility addresses pursuant to information shared by California DOJ.
