What the inspector wrote

On 10/24/2025, Licensing Program Analyst (LPA) Amelia Morales conducted an unannounced case management inspection due to an incident which occurred on 08/04/2025. The written report was mailed and received to the Regional Office on 8/06/2025. LPA arrived at the facility and met with Director Leslie Chiang, who guided LPA on a tour of the facility. Purple Room: 9 preschool children with a ratio of 1 teacher Yellow Room: 13 preschool children with a ratio of 2 teacher Blue Room: 9 preschool children with a ratio of 2 teachers TK Room: 12 preschool children with a ratio of 1 teachers The incident, which occurred, Child #1 tripped and fell landing on their chin. Child #1 was present during the visit. During staff interviews LPA was informed that Staff #1 had active supervision of the child who obtained an injury. LPA Morales inspected where the incident occurred, LPA Morales took photos of where the incident occurred. LPA Morales took photos of the toy truck Child #1 was carrying when the incident occurred. Page 1 of 2 LPA Morales was informed by Staff #1, Staff #2, and staff#3,\302\240that Child #1 lost their footing while carrying a toy truck. Per Staff #1, Child #1 did not have his footing all the way and could not see the curb (step). Per Staff #1, Child #1 lost their footing, and fell forward, making contact with their chin on the ground. When asked if Child #1 was walking or running, Staff #1 stated Child #1 was walking. Staff #1 informed LPA that they checked on Child #1, and noticed Child #1 was bleeding from their mouth. Per Staff #1, Child #1 was taken to the Administration office, was provided ice for injury. Per Staff #3 parents were notified of the incident. Per Staff #3, Child #1 was picked up during regular pick up time. Per Staff #3, Child #1 was taken to the doctors later that day by parents due to unusual behavior and vomiting. When asked if Child #1 had a concussion Staff #2 and Staff #3 stated "No" Per Staff #3, "it was suspected but not confirmed." During Child #1 interview, Child #1 informed LPA they "fell down on side walk," (step on the playground) During the inspection, LPA obtained a copy of the children's roster, obtained a copy of internal "Ouch Reports," conducted interviews with Staff, interviewed Child #1, took photos and made observations of where the incident occurred. At this time, no further investigation is needed. There are no deficiencies observed in regards to today's visit. Notice of Site was given and must remain posted for 30 days. Exit interview conducted and report was reviewed with Director Leslie Chiang. Page 2 of 2
